perf_counter: fix counter freeing logic

Fix counter lifetime bugs which explain the crashes reported by
Marcelo Tosatti and Arnaldo Carvalho de Melo.

The new rule is: flushing + freeing is only done for a task's
own counters, never for other tasks.
