staging: comedi: s626: add missing bits for 'CRB' register
There are some bits in the 'CRB' register not defined in "s626.h".
Three of these are read-only bits that overlay the write-only interrupt
control bits. Another missing bit controls whether counter 'B' is
cleared when counter 'A' overflows. Add the missing bit definitions for
completeness.

staging: comedi: s626: correct S626_CRAMSK_CLKPOL_A macro (unused)
The counter 'A' clock polarity field in the 'CRA' register is only 1 bit
wide, but the `S626_CRAMSK_CLKPOL_A` macro shows it as 2 bits wide,
which would overlap with the counter 'A' interrupt source field. This
is harmless as the macro isn't actually used yet, but correct it anyway
as I want to use it!

staging: comedi: s626: distinguish counter src from encoder mode
The macros `S626_CLKSRC_COUNTER`, `S626_CLKSRC_TIMER` and
`S626_CLKSRC_EXTENDER` are used logically to set the operating mode of
an encoder channel. `S626_CLKSRC_COUNTER` (0) is also used as a 2-bit
physical value to set the counter source of an encoder channel to
"encoder".
Rename the macros to `S626_ENCMODE_COUNTER`, `S626_ENCMODE_TIMER` and
`S626_ENCMODE_EXTENDER` and rename some other macros and (unused)
functions relating to the encoder mode for consistency.
Define new macros to specify the physical counter source values for the
'CRA' register and rename the corresponding bitshift and mask macros
accordingly. The physical values for the counter source are:
S626_CNTSRC_ENCODER = 0 // encoder
S626_CNTSRC_DIGIN = 1 // digital inputs
S626_CNTSRC_SYSCLK = 2 // system clock up
S626_CNTSRC_SYSCLK_DOWN = 3 // system clock down
Also use the `S626_CNTSRC_SYSCLK` value as a bitmask (bit 1) to indicate
either of the system clock values, with the direction (bit 0) indicated
separately in this case.

When setting the clock source for one of the 'A' encoders to operate in
"counter" mode in `s626_set_mode_a()`, bitshift the clock source value by
`S626_CRABIT_CLKSRC_A` for consistency with the other modes. This has
no effect on the value since `S626_CRABIT_CLKSRC_A` is 0.

When setting up an encoder channel, the setup value includes a polarity
and direction, but these are the same bit of the setup value:
S626_CLKPOL_POS = S626_CNTDIR_UP = 0
S626_CLKPOL_NEG = S626_CNTDIR_DOWN = 1
In the construction of the setup value, both the CLKPOL and the CNTDIR
constants are shifted by the same amount `S626_BF_CLKPOL`. Only the
following combinations are set up currently (this may change if user
configuration of the encoder is implemented properly):
(S626_CLKPOL_POS << S626_BF_CLKPOL)
(S626_CLKPOL_POS << S626_BF_CLKPOL) |
(S626_CNTDIR_UP << S626_BF_CLKPOL)
(S626_CLKPOL_POS << S626_BF_CLKPOL) |
(S626_CNTDIR_DOWN << S626_BF_CLKPOL)
The first two are used in "counter" mode and is equivalent to:
(S626_CLKPOL_POS << S626_BF_CLKPOL)
The last one is used in "timer" mode and is equivalent to:
(S626_CNTDIR_DOWN << S626_BF_CLKPOL)
Use the shorter equivalents. The comments in "s626.h" indicate that the
'CLKPOL' constants make more sense for the "counter" mode (when the
encoders operate as up/down counters) and the 'CNTDIR' constants make
more sense for the "timer" mode.

staging: comedi: rtd520: use normal bitfield functions
The `unsigned char chan_is_bipolar[]` member of `struct rtd_private` is
used with some macros as a packed array of 1-bit values that indicate
whether the corresponding entries in the hardware's "channel-gain" table
have been set to a bipolar (1) or unipolar (0) range, as the raw samples
from the hardware need to be cooked differently in each case.
Replace the declaration of the member with a standard Linux bitfield
using `DECLARE_BITFIELD()`, and replace the home-grown macros used
access the bitfield with the standard Linux non-atomic bitop functions,
`__set_bit()`, `__clear_bit()` and `test_bit()`.

Core improvements
* Some more fixes and cleanups related to buffers. These include the second
half of a series which went is as fixes. The basis for delaying until the
next merge window is that some are too invasive for this late in a cycle
and others only effect code paths current unused in the mainline tree.
In this case we have:
* protecting against concurrent userspace access
* fixing a memory leak if a device goes away
* avoiding always reallocating the buffer whether or not it has changed
(a bug fix, but one with no functional changes other than a small speed
improvement.)
* Add reference counting for buffers to ensure they hang around if open
from userspace or in kernel when the device is forcefully removed.
* Return -ENODEV for buffer access operations when the device has gone
away.
* Add proper locking for iio_update_buffers (currently we only have one
buffer per device in mainline, but an input bridge driver is under
development which would make this bug 'real'.)
* Wake up anyone waiting on a buffer if the device is unregistered. A
subsequent read will fail, notifying userspace that the device is no
longer there rather than having it wait possibly for ever.
* Move the iio_sw_preenable functionality into the core. This avoids drivers
having to 'know' about how the buffers are implemented and is called by
almost all drivers anyway. Those that don't call it are not harmed by it
being called.
* New registration approach for information (i.e. sysfs attributes) about
events. Much more generic and now similar to how the equivalent is
handled for channel information. The events infrastructure had been left
behind by other changes so this brings it back in line.
* Using the new events registration approach, add a hysterisis event_info
element and apply this to those drivers with this property.
* A little unitialized variable bug in the generic_buffer.c example.
* Factor out the code for freeing lists of IIO Device attributes to avoid
some repitition.
Driver cleanups
* At91 driver gains touch screen support and some related fixes.
* Follow up series of patches removing the now redundant
iio_sw_buffer_preenable calls.
* Lots of conversions to the new event registration methods.
* Another round of hmc5843 cleanups as that driver moves towards graduating
from staging.
* Make some SoC drivers buildable if COMPILE_TEST is used. Follow up fixes
for a few bits and bobs that revealed.
* Add explicit includes of linux/of.h to those drivers making us of linux/of.h

iio: at91: introduce touch screen support in iio adc driver
AT91 ADC hardware integrate touch screen support. So this patch add touch
screen support for at91 adc iio driver.
To enable touch screen support in adc, you need to add the dt parameters:
1. which type of touch are used? (4 or 5 wires), sample period time.
2. correct pressure detect threshold value.
In the meantime, since touch screen will use a interal period trigger of adc,
so it is conflict to other hardware triggers. Driver will disable the hardware
trigger support if touch screen is enabled.
This driver has been tested in AT91SAM9X5-EK and SAMA5D3x-EK.

changed the conditional around the declaration of usb_nop_xceiv_register
from

(defined(CONFIG_NOP_USB_XCEIV_MODULE) && defined(MODULE))
to
#if IS_ENABLED(CONFIG_NOP_USB_XCEIV)
While that looks the same, it is semantically different. The first expression
is true if CONFIG_NOP_USB_XCEIV is built as module and if the including
code is built as module. The second expression is true if code depending on
CONFIG_NOP_USB_XCEIV if built as module or into the kernel.
As a result, the arm:allmodconfig build fails with
arch/arm/mach-omap2/built-in.o: In function `omap3_evm_init':
arch/arm/mach-omap2/board-omap3evm.c:703: undefined reference to
`usb_nop_xceiv_register'
Fix the problem by reverting to the old conditional.

ACPI / PM: Drop two functions that are not used any more
Two functions defined in device_pm.c, acpi_dev_pm_add_dependent()
and acpi_dev_pm_remove_dependent(), have no callers and may be
dropped, so drop them.
Moreover, they are the only functions adding entries to and removing
entries from the power_dependent list in struct acpi_device, so drop
that list too.

ATA / ACPI: remove power dependent device handling
Previously, we wanted SCSI devices corrsponding to ATA devices to
be runtime resumed when the power resource for those ATA device was
turned on by some other device, so we added the SCSI device to the
dependent device list of the ATA device's ACPI node. However, this
code has no effect after commit
41863fc (ACPI / power: Drop automaitc
resume of power resource dependent devices) and the mechanism it was
supposed to implement is regarded as a bad idea now, so drop it.

mm: revert mremap pud_free anti-fix
Revert commit
1ecfd533f4c5 ("mm/mremap.c: call pud_free() after fail
calling pmd_alloc()").
The original code was correct: pud_alloc(), pmd_alloc(), pte_alloc_map()
ensure that the pud, pmd, pt is already allocated, and seldom do they
need to allocate; on failure, upper levels are freed if appropriate by
the subsequent do_munmap(). Whereas commit
1ecfd533f4c5 did an
unconditional pud_free() of a most-likely still-in-use pud: saved only
by the near-impossiblity of pmd_alloc() failing.

mm: fix BUG in __split_huge_page_pmd
Occasionally we hit the BUG_ON(pmd_trans_huge(*pmd)) at the end of
__split_huge_page_pmd(): seen when doing madvise(,,MADV_DONTNEED).
It's invalid: we don't always have down_write of mmap_sem there: a racing
do_huge_pmd_wp_page() might have copied-on-write to another huge page
before our split_huge_page() got the anon_vma lock.
Forget the BUG_ON, just go back and try again if this happens.

swap: fix set_blocksize race during swapon/swapoff
Fix race between swapoff and swapon. Swapoff used old_block_size from
swap_info outside of swapon_mutex so it could be overwritten by
concurrent swapon.
The race has visible effect only if more than one swap block device
exists with different block sizes (e.g. /dev/sda1 with block size 4096
and /dev/sdb1 with 512). In such case it leads to setting the blocksize
of swapped off device with wrong blocksize.
The bug can be triggered with multiple concurrent swapoff and swapon:
0. Swap for some device is on.
1. swapoff:
First the swapoff is called on this device and "struct swap_info_struct
*p" is assigned. This is done under swap_lock however this lock is
released for the call try_to_unuse().
2. swapon:
After the assignment above (and before acquiring swapon_mutex &
swap_lock by swapoff) the swapon is called on the same device.
The p->old_block_size is assigned to the value of block_size the device.
This block size should be the same as previous but sometimes it is not.
The swapon ends successfully.
3. swapoff:
Swapoff resumes, grabs the locks and mutex and continues to disable this
swap device. Now it sets the block size to value taken from swap_info
which was overwritten by swapon in 2.

procfs: call default get_unmapped_area on MMU-present architectures
Commit
c4fe24485729 ("sparc: fix PCI device proc file mmap(2)") added
proc_reg_get_unmapped_area in proc_reg_file_ops and
proc_reg_file_ops_no_compat, by which now mmap always returns EIO if
get_unmapped_area method is not defined for the target procfs file,
which causes regression of mmap on /proc/vmcore.
To address this issue, like get_unmapped_area(), call default
current->mm->get_unmapped_area on MMU-present architectures if
pde->proc_fops->get_unmapped_area, i.e. the one in actual file
operation in the procfs file, is not defined.

procfs: fix unintended truncation of returned mapped address
Currently, proc_reg_get_unmapped_area truncates upper 32-bit of the
mapped virtual address returned from get_unmapped_area method in
pde->proc_fops due to the variable rv of signed integer on x86_64. This
is too small to have vitual address of unsigned long on x86_64 since on
x86_64, signed integer is of 4 bytes while unsigned long is of 8 bytes.
To fix this issue, use unsigned long instead.
Fixes a regression added in commit
c4fe24485729 ("sparc: fix PCI device
proc file mmap(2)").

bounded by max_pause. It's suspected and demonstrated that the
max_pause calculation goes wrong:

The problem lies in the two "long = unsigned long" assignments in
bdi_max_pause() which might go negative if the highest bit is 1, and the

fs: buffer: move allocation failure loop into the allocator
Buffer allocation has a very crude indefinite loop around waking the
flusher threads and performing global NOFS direct reclaim because it can
not handle allocation failures.
The most immediate problem with this is that the allocation may fail due
to a memory cgroup limit, where flushers + direct reclaim might not make
any progress towards resolving the situation at all. Because unlike the
global case, a memory cgroup may not have any cache at all, only
anonymous pages but no swap. This situation will lead to a reclaim
livelock with insane IO from waking the flushers and thrashing unrelated
filesystem cache in a tight loop.
Use __GFP_NOFAIL allocations for buffers for now. This makes sure that
any looping happens in the page allocator, which knows how to
orchestrate kswapd, direct reclaim, and the flushers sensibly. It also
allows memory cgroups to detect allocations that can't handle failure
and will allow them to ultimately bypass the limit if reclaim can not
make progress.

mm: memcg: handle non-error OOM situations more gracefully
Commit
3812c8c8f395 ("mm: memcg: do not trap chargers with full
callstack on OOM") assumed that only a few places that can trigger a
memcg OOM situation do not return VM_FAULT_OOM, like optional page cache
readahead. But there are many more and it's impractical to annotate
them all.
First of all, we don't want to invoke the OOM killer when the failed
allocation is gracefully handled, so defer the actual kill to the end of
the fault handling as well. This simplifies the code quite a bit for
added bonus.
Second, since a failed allocation might not be the abrupt end of the
fault, the memcg OOM handler needs to be re-entrant until the fault
finishes for subsequent allocation attempts. If an allocation is
attempted after the task already OOMed, allow it to bypass the limit so
that it can quickly finish the fault and invoke the OOM killer.

block/partitions/efi.c: treat size mismatch as a warning, not an error
In commit
27a7c642174e ("partitions/efi: account for pmbr size in lba")
we started treating bad sizes in lba field of the partition that has the
0xEE (GPT protective) as errors.
However, we may run into these "bad sizes" in the real world if someone
uses dd to copy an image from a smaller disk to a bigger disk. Since
this case used to work (even without using force_gpt), keep it working
and treat the size mismatch as a warning instead of an error.

mm: hugetlb: initialize PG_reserved for tail pages of gigantic compound pages
Commit
11feeb498086 ("kvm: optimize away THP checks in
kvm_is_mmio_pfn()") introduced a memory leak when KVM is run on gigantic
compound pages.
That commit depends on the assumption that PG_reserved is identical for
all head and tail pages of a compound page. So that if get_user_pages
returns a tail page, we don't need to check the head page in order to
know if we deal with a reserved page that requires different
refcounting.
The assumption that PG_reserved is the same for head and tail pages is
certainly correct for THP and regular hugepages, but gigantic hugepages
allocated through bootmem don't clear the PG_reserved on the tail pages
(the clearing of PG_reserved is done later only if the gigantic hugepage
is freed).
This patch corrects the gigantic compound page initialization so that we
can retain the optimization in
11feeb498086. The cacheline was already
modified in order to set PG_tail so this won't affect the boot time of
large memory systems.

thread 0: now call zswap_get_swap_cache_page
swapcache_prepare return -ENOENT because entry x is not used any more
zswap_get_swap_cache_page return ZSWAP_SWAPCACHE_NOMEM
zswap_writeback_entry do nothing except put refcount
Now, the memory of zswap_entry x and its zpage leak.
Modify:
- check the refcount in fail path, free memory if it is not referenced.
- use ZSWAP_SWAPCACHE_FAIL instead of ZSWAP_SWAPCACHE_NOMEM as the fail path
can be not only caused by nomem but also by invalidate.

If a page we are inspecting is in swap we may occasionally report it as
having soft dirty bit (even if it is clean). The pte_soft_dirty helper

mm: migration: do not lose soft dirty bit if page is in migration state
If page migration is turned on in config and the page is migrating, we
may lose the soft dirty bit. If fork and mprotect are called on
migrating pages (once migration is complete) pages do not obtain the
soft dirty bit in the correspond pte entries. Fix it adding an
appropriate test on swap entries.

mm/hugetlb.c: correct missing private flag clearing
We should clear the page's private flag when returing the page to the
hugepage pool. Otherwise, marked hugepage can be allocated to the user
who tries to allocate the non-reserved hugepage. If this user fail to
map this hugepage, he would try to return the page to the hugepage pool.
Since this page has a private flag, resv_huge_pages would mistakenly
increase. This patch fixes this situation.
