drm/i915: drop duplicate ggtt vma list add in setup_global_gtt
Preallocated objects will already have been added to the vma_list when
creating their ggtt vma entry, and coincidentally also marked as holding
a ggtt mapping. Repeating the vma_list manipulation when setting up the
ggtt after preallocation is a recipe for an unhappy kernel.

The struct returned contains context loss counters.

drm/i915: add i915_reset_count
reset_counter will be incremented twice per successful
reset. Odd values mean reset is in progress and even values
mean that reset has completed.
Reset status ioctl introduced in following commit
needs to deliver global reset count to userspace so
use reset_counter to derive the actual reset count
for the gpu
Note that reset in progress is enough to increment
the counter.

vlv_dpio_read/write should be describe more in PHY centric instead of
display controller centric.
Create a enum dpio_channel for channel index and enum dpio_phy for PHY

drm/i915: Make AGP support optional
We only depend on the intel-gtt module for GTT frobbign on older gens.
The intel_agp module is optional, except for UMS and some old XvMC
userland on gen3. So make AGP support optional. As before, we will
fail the i915 init for UMS and gen3 KMS the same as before if
intel_agp isn't around.
intel-gtt.c is left with a somewhat ugly ifdef mess, but I'm going
to save that for a later cleaning.
At least my gen2 still works with the patch and CONFIG_AGP=n.

drm/i915/vlv: Rename VLV DPIO register to be more structure to match configdb document.
Some VLV PHY/PLL DPIO registers have group/lane/channel access. Current
DPIO register definition doesn't have a structure way to break them
down. As a result it is not easy to match the PHY/PLL registers with the
configdb document. Rename those registers based on the configdb for easy
cross references, and without the need to check the offset in the header
file.
New format is as following.
<platform name>_<DPIO component><optional lane #>_DW<dword # in the
doc>_<optional channel #>
For example,
VLV_PCS_DW0 - Group access to PCS for lane 0 to 3 for PCS DWORD 0.
VLV_PCS01_DW0_CH0 - PCS access to lane 0/1, channel 0 for PCS DWORD 0.
Another example is
VLV_TX_DW0 - Group access to TX lane 0 to 3 for TX DWORD 0
VLV_TX0_DW0 - Refer to TX Lane 0 access only for TX DWORD 0.
There is no functional change on this patch.

drm/i915: Kill vlv_update_rps_cur_delay()
Polling to make sure the current GPU frequency matches the last
requested frequency should not be necessay, and if there's some
throttling involved, the two might not match anyway.
Since we're still seeing this trigger occasionally, and it just
introduces a rather pointless 10 ms delay, it seems like better
to kill it off.

drm/i915: Initialise min/max frequencies before updating RPS registers
The RPS register writing routines use the current value of min/max to
set certain limits and interrupt gating. If we set those afterwards, we
risk setting up the hw incorrectly and losing power management events,
and worse, trigger some internal assertions.
Reorder the calling sequences to be correct, and remove the then
unrequired clamping from inside set_rps(). And for a bonus, fix the bug
of calling gen6_set_rps() from Valleyview.

drm/i915/vlv: For i915_cur_delayinfo, the max frequency reporting wrong value.
The max frequency reporting is not correct. But there is already an existing
valleyview_rps_max_freq and valleyview_rps_min_freq to get the
frequency. Use that for i915_cur_delayinfo.

drm/i915: Fix gen3/4 vblank counter wraparound
When the hardware frame counter reads 0xffffff and we're already past
vblank start, we'd return 0x1000000 as the vblank counter value. Once
we'd cross into the next frame's active portion, the vblank counter
would wrap to 0. So we're reporting two different vblank counter values
for the same frame.
Fix the problem by masking the cooked value by 0xffffff to make sure
the counter wraps already after vblank start.

drm/i915/vlv: Workaround a punit issue in DDR data rate for 1333.
For DDR data rate reporting by Punit in PUNIT_GPU_FREQ_STS, the actual
data encoding is 00b=800, 01b=1066, 10b=1333, 11b=1333.
Some premium VLV sku will get the DDR_DATA_RATE set as 11. As a result,
the turbo frequency reporting will be incorrect without this workaround.

drm/i915: Improve vlv_gpu_freq() and vlv_freq_opcode()
We're currently miscalculating the VLV graphics clock a little bit.
This is caused by rounding the step to integer MHz, which does not
match reality. Change the formula to match the GUnit HAS to give
more accurate answers.

It's possible that the CCK clock could run at a different rate than the
DDR clock, so use the same method to get CCK as the GMBUS code does when

On VLV/BYT, we can adjust the CDclk frequency up or down based on the
max pixel clock we need to drive. Lowering it can save power, while
raising it is necessary to support high resolution.
Add a new callback in modeset_affected_pipes and a
modeset_global_resources function to perform this adjustment as
necessary.

drm/i915/ns2501: Rip out the reenable hack
With the change in the modeset sequence this shouldn't be required
any more since the ->mode_set callback now gets called when the dvo
port is fully up and running.
Also limit the retry loop to 10 tries to avoid hanging the machine
while holding important modeset locks.

drm/i915/dvo: call ->mode_set callback only when the port is running
The ns2501 controller seems to need the dpll and dvo port to accept
the timing update commands. Quick testing on my x30 here seems to
indicate that other dvo controllers don't mind. So let's move the
->mode_set callback to a place where we have the port up and running
already.

Merge tag 'v3.12' into drm-intel-next
I want to merge in the new Broadwell support as a late hw enabling
pull request. But since the internal branch was based upon our
drm-intel-nightly integration branch I need to resolve all the
oustanding conflicts in drm/i915 with a backmerge to make the 60+
patches apply properly.
We'll propably have some fun because Linus will come up with a
slightly different merge solution.

ipc, msg: forbid negative values for "msg{max,mnb,mni}"
Negative message lengths make no sense -- so don't do negative queue
lenghts or identifier counts. Prevent them from getting negative.
Also change the underlying data types to be unsigned to avoid hairy
surprises with sign extensions in cases where those variables get
evaluated in unsigned expressions with bigger data types, e.g size_t.
In case a user still wants to have "unlimited" sizes she could just use
INT_MAX instead.

ARC: Incorrect mm reference used in vmalloc fault handler
A vmalloc fault needs to sync up PGD/PTE entry from init_mm to current
task's "active_mm". ARC vmalloc fault handler however was using mm.
A vmalloc fault for non user task context (actually pre-userland, from
init thread's open for /dev/console) caused the handler to deref NULL mm
(for mm->pgd)
The reasons it worked so far is amazing:
1. By default (!SMP), vmalloc fault handler uses a cached value of PGD.
In SMP that MMU register is repurposed hence need for mm pointer deref.
2. In pre-3.12 SMP kernel, the problem triggering vmalloc didn't exist in
pre-userland code path - it was introduced with commit

scripts/kallsyms: filter symbols not in kernel address space
This patch uses CONFIG_PAGE_OFFSET to filter symbols which
are not in kernel address space because these symbols are
generally for generating code purpose and can't be run at
kernel mode, so we needn't keep them in /proc/kallsyms.
For example, on ARM there are some symbols which may be
linked in relocatable code section, then perf can't parse
symbols any more from /proc/kallsyms, this patch fixes the
problem (introduced

Pull USB fixes from Greg KH:
"Here is a set of patches that revert all of the changes done to the
pl2303 USB serial driver in the 3.12-rc timeframe, as it turns out
they break some devices that work just fine on 3.11. As it's not a
good idea to break working systems, drop them all and they will be
reworked for future kernel versions such that there is no breakage.
I've also included a MAINTAINERS update for the USB serial subsystem
and a new device id for the ftdi_sio driver as well"

memcg: remove incorrect underflow check
When a memcg is deleted mem_cgroup_reparent_charges() moves charged
memory to the parent memcg. As of
v3.11-9444-g3ea67d0 "memcg: add per
cgroup writeback pages accounting" there's bad pointer read. The goal
was to check for counter underflow. The counter is a per cpu counter
and there are two problems with the code:
(1) per cpu access function isn't used, instead a naked pointer is used
which easily causes oops.
(2) the check doesn't sum all cpus
Test:
$ cd /sys/fs/cgroup/memory
$ mkdir x
$ echo 3 > /proc/sys/vm/drop_caches
$ (echo $BASHPID >> x/tasks && exec cat) &
[1] 7154
$ grep ^mapped x/memory.stat
mapped_file 53248
$ echo 7154 > tasks
$ rmdir x
<OOPS>
The fix is to remove the check. It's currently dangerous and isn't
worth fixing it to use something expensive, such as
percpu_counter_sum(), for each reparented page. __this_cpu_read() isn't
enough to fix this because there's no guarantees of the current cpus
count. The only guarantees is that the sum of all per-cpu counter is >=
nr_pages.

drm/i915: avoid unclaimed registers when capturing the error state
Even though we only check for unclaimed registers while we're writing
registers, if we read a bad register we'll still trigger a CPU error
interrupt, and we'll print an "Unclaimed register" DRM_ERROR due to
that. To avoid this error, just avoid touching power domains that are
not enabled.
Use kzalloc so we're sure all the disabled domains will be zeroed on
the error state file. We already print the information that is enough
to discover if the power well is enabled on the error state file, so
this should not be a problem.

drm/i915: scramble reset support for DP port CRC on g4x
We need to reset the DP scrambler on every vsync to get stable CRCs.
And since we can't use the normal pipe CRC on DP ports on g4x we
really need them to be able to test modesetting issues on (e)DP
outputs.
Note that the DC balance reset is for SDVO port CRCs so we don't
strictly need it. But better safe than sorry (and it's a nice template
in case we ever want to grab port CRCs for e.g. audio checking).

drm/i916: add "auto" pipe CRC source
On gmch platforms the normal pipe source CRC registers don't work for
DP and TV encoders. And on newer platforms the single pipe CRC has
been replaced by a set of CRC at different stages in the platform.
Now most of our userspace tests don't care one bit about the exact
CRC, they simply want something that reflects any changes on the
screen. Hence add a new auto target for platform agnostic tests to
use.
v2: Pass back the adjusted source so that it can be shown in debugfs.
v3: I seem to be unable to get a stable CRC for DP ports. So let's
just disable them for now when using the auto mode. Note that
testcases need to be restructured so that they can dynamically skip
connectors. They also first need to set up the desired mode
configuration, since otherwise the auto mode won't do the right thing.
v4: Don't leak the modeset mutex on error paths.

USB: Maintainers change for usb serial drivers
Johan has been conned^Wgracious in accepting the maintainership of the
USB serial drivers, especially as he's been doing all of the real work
for the past few years.
At the same time, remove a bunch of old entries for USB serial drivers
that don't make sense anymore, given that the developers are no longer
around, and individual driver maintainerships for tiny things like this
is pretty pointless.

lib/scatterlist.c: don't flush_kernel_dcache_page on slab page
Commit
b1adaf65ba03 ("[SCSI] block: add sg buffer copy helper
functions") introduces two sg buffer copy helpers, and calls
flush_kernel_dcache_page() on pages in SG list after these pages are
written to.
Unfortunately, the commit may introduce a potential bug:
- Before sending some SCSI commands, kmalloc() buffer may be passed to
block layper, so flush_kernel_dcache_page() can see a slab page
finally
- According to cachetlb.txt, flush_kernel_dcache_page() is only called
on "a user page", which surely can't be a slab page.
- ARCH's implementation of flush_kernel_dcache_page() may use page
mapping information to do optimization so page_mapping() will see the
slab page, then VM_BUG_ON() is triggered.
Aaro Koskinen reported the bug on ARM/kirkwood when DEBUG_VM is enabled,
and this patch fixes the bug by adding test of '!PageSlab(miter->page)'
before calling flush_kernel_dcache_page().

mm: memcg: fix test for child groups
When memcg code needs to know whether any given memcg has children, it
uses the cgroup child iteration primitives and returns true/false
depending on whether the iteration loop is executed at least once or
not.
Because a cgroup's list of children is RCU protected, these primitives
require the RCU read-lock to be held, which is not the case for all
memcg callers. This results in the following splat when e.g. enabling

In the memcg case, we only care about children when we are attempting to
modify inheritable attributes interactively. Racing with deletion could
mean a spurious -EBUSY, no problem. Racing with addition is handled
just fine as well through the memcg_create_mutex: if the child group is
not on the list after the mutex is acquired, it won't be initialized
from the parent's attributes until after the unlock.

mm: memcg: use proper memcg in limit bypass
Commit
84235de394d9 ("fs: buffer: move allocation failure loop into the
allocator") allowed __GFP_NOFAIL allocations to bypass the limit if they
fail to reclaim enough memory for the charge. But because the main test
case was on a 3.2-based system, the patch missed the fact that on newer
kernels the charge function needs to return root_mem_cgroup when
bypassing the limit, and not NULL. This will corrupt whatever memory is
at NULL + percpu pointer offset. Fix this quickly before problems are
reported.

vfs: decrapify dput(), fix cache behavior under normal load
We do not want to dirty the dentry->d_flags cacheline in dput() just to
set the DCACHE_REFERENCED flag when it is already set in the common case
anyway. This way the first cacheline of the dentry (which contains the
RCU lookup information etc) can stay shared among multiple CPU's.
This finishes off some of the details of all the scalability patches
merged during the merge window.
Also don't mark dentry_kill() for inlining, since it's the uncommon path
and inlining it just makes the common path slower due to extra function
entry/exit overhead.

This is provoked when the ASoC front end is open along with its backend,
(which causes the backend to have a runtime assigned to it) and then the
SNDRV_CTL_IOCTL_PCM_INFO is requested for the (visible) backend device.
Resolve this by ensuring that ASoC internal backend devices are not
visible to userspace, just as the commentry for snd_pcm_new_internal()
says it should be.

drm/i915: reduce eDP VDD message verbose
Now we only print messages when we actually enable VDD and when we
actually disable VDD.
The changes in the last commit triggered a big number of messages
while the driver was being initialized, and I thought we were toggling
things on/off too many times, but that was not really true: we were
just being too verbose.

drm/i915: turn the eDP VDD on for any i2c transactions
If the eDP output is disabled, then we try to use /dev/i2c-X file to
do i2c transations, we get a WARN from intel_dp_check_edp() saying
we're trying to do AUX communication with the panel off. So this
commit reorganizes the code so we enable the VDD at
intel_dp_i2c_aux_ch() instead of just the callers inside i915.ko.
This fixes the i2c subtest from the pc8 test of intel-gpu-tools on
machines that have eDP panels.

memcg: use __this_cpu_sub() to dec stats to avoid incorrect subtrahend casting
As of commit
3ea67d06e467 ("memcg: add per cgroup writeback pages
accounting") memcg counter errors are possible when moving charged
memory to a different memcg. Charge movement occurs when processing
writes to memory.force_empty, moving tasks to a memcg with
memcg.move_charge_at_immigrate=1, or memcg deletion.
An example showing error after memory.force_empty:

This issue only affects the source memcg on 64 bit machines; the
destination memcg counters are correct. So the rmdir case is not too
important because such counters are soon disappearing with the entire
memcg. But the memcg.force_empty and memory.move_charge_at_immigrate=1
cases are larger problems as the bogus counters are visible for the
(possibly long) remaining life of the source memcg.
The problem is due to memcg use of __this_cpu_from(.., -nr_pages), which
is subtly wrong because it subtracts the unsigned int nr_pages (either
-1 or -512 for THP) from a signed long percpu counter. When
nr_pages=-1, -nr_pages=0xffffffff. On 64 bit machines stat->count[idx]
is signed 64 bit. So memcg's attempt to simply decrement a count (e.g.
from 1 to 0) boils down to:

unsigned int nr_pages = 1
count += -nr_pages /* -nr_pages == 0xffff,ffff */
count is now 0x1,0000,0000 instead of 0
The fix is to subtract the unsigned page count rather than adding its
negation. This only works once "percpu: fix this_cpu_sub() subtrahend
casting for unsigneds" is applied to fix this_cpu_sub().

percpu: fix this_cpu_sub() subtrahend casting for unsigneds
this_cpu_sub() is implemented as negation and addition.
This patch casts the adjustment to the counter type before negation to
sign extend the adjustment. This helps in cases where the counter type
is wider than an unsigned adjustment. An alternative to this patch is
to declare such operations unsupported, but it seemed useful to avoid
surprises.
This patch specifically helps the following example:
unsigned int delta = 1
preempt_disable()
this_cpu_write(long_counter, 0)
this_cpu_sub(long_counter, delta)
preempt_enable()
Before this change long_counter on a 64 bit machine ends with value
0xffffffff, rather than 0xffffffffffffffff. This is because
this_cpu_sub(pcp, delta) boils down to this_cpu_add(pcp, -delta),
which is basically:
long_counter = 0 + 0xffffffff
Also apply the same cast to:

mm/pagewalk.c: fix walk_page_range() access of wrong PTEs
When walk_page_range walk a memory map's page tables, it'll skip
VM_PFNMAP area, then variable 'next' will to assign to vma->vm_end, it
maybe larger than 'end'. In next loop, 'addr' will be larger than
'next'. Then in /proc/XXXX/pagemap file reading procedure, the 'addr'
will growing forever in pagemap_pte_range, pte_to_pagemap_entry will

mm: list_lru: fix almost infinite loop causing effective livelock
I've seen a fair number of issues with kswapd and other processes
appearing to get stuck in v3.12-rc. Using sysrq-p many times seems to
indicate that it gets stuck somewhere in list_lru_walk_node(), called
from prune_icache_sb() and super_cache_scan().
I never seem to be able to trigger a calltrace for functions above that

So, if *nr_to_walk was zero when this function was entered, that means
we're wanting to operate on (~0UL)+1 objects - which might as well be
infinite.
Clearly this is not correct behaviour. If we think about the behaviour
of this function when *nr_to_walk is 1, then clearly it's wrong - we
decrement first and then test for zero - which results in us doing
nothing at all. A post-decrement would give the desired behaviour -
we'd try to walk one object and one object only if *nr_to_walk were one.
It also gives the correct behaviour for zero - we exit at this point.

staging: wlags49_h2: buffer overflow setting station name
We need to check the length parameter before doing the memcpy(). I've
actually changed it to strlcpy() as well so that it's NUL terminated.
You need CAP_NET_ADMIN to trigger these so it's not the end of the
world.

MIPS: malta: Fix GIC interrupt offsets
The GIC interrupt offsets are calculated based on the value of NR_CPUS.
However, this is wrong because NR_CPUS may or may not contain the real
number of the actual cpus present in the system. We fix that by using
the 'nr_cpu_ids' variable which contains the real number of cpus in
the system. Previously, an MT core (eg with 8 VPEs) will fail to boot if
NR_CPUS was > 8 with the following errors:

This is now fixed and the Malta board can boot with any NR_CPUS value
which also helps supporting more processors in a single kernel binary.

Commit
2dc4128 (ACPI / hotplug / PCI: Avoid doing too much for
spurious notifies) changed the enable_slot() to check return value of
pci_scan_slot() and if it is zero return early from the function. It
means that there were no new devices in this particular slot.
However, if a device appeared deeper in the hierarchy the code now
ignores it causing things like Thunderbolt chaining fail to recognize
new devices.
The problem with Alex Williamson's machine was solved with commit
a47d8c8 (ACPI / hotplug / PCI: Avoid parent bus rescans on spurious
device checks) and hence we should be able to restore the original
functionality that we always rescan on bus check notification.
On a device check notification we still check what acpiphp_rescan_slot()
returns and on zero bail out early.

32-bit x86 machines. Paul says:

call) resolves those issues.
Resuming from suspend on i686 on (release candidates of) v3.11 and

which is reported to cause user space memory corruption to happen
after suspend to RAM.
Since it appears to be extremely difficult to root cause this
problem, it is best to revert the offending commit and try to address
the original issue in a better way later.

KVM: Fix modprobe failure for kvm_intel/kvm_amd
The x86 specific kvm init creates a new conflicting
debugfs directory which causes modprobe issues
with kvm_intel and kvm_amd. For example,
sudo modprobe kvm_amd
modprobe: ERROR: could not insert 'kvm_amd': Bad address
The simplest fix is to just rename the directory. The following
