x86-64, modify_ldt: Ban 16-bit segments on 64-bit kernels
The IRET instruction, when returning to a 16-bit segment, only
restores the bottom 16 bits of the user space stack pointer. We have
a software workaround for that ("espfix") for the 32-bit kernel, but
it relies on a nonzero stack segment base which is not available in
32-bit mode.
Since 16-bit support is somewhat crippled anyway on a 64-bit kernel
(no V86 mode), and most (if not quite all) 64-bit processors support
virtualization for the users who really need it, simply reject
attempts at creating a 16-bit segment when running on top of a 64-bit
kernel.

efi: Pass correct file handle to efi_file_{read,close}
We're currently passing the file handle for the root file system to
efi_file_read() and efi_file_close(), instead of the file handle for the
file we wish to read/close.
While this has worked up until now, it seems that it has only been by
pure luck. Olivier explains,
"The issue is the UEFI Fat driver might return the same function for
'fh->read()' and 'h->read()'. While in our case it does not work with
a different implementation of EFI_SIMPLE_FILE_SYSTEM_PROTOCOL. In our
case, we return a different pointer when reading a directory and
reading a file."
Fixing this actually clears up the two functions because we can drop one
of the arguments, and instead only pass a file 'handle' argument.

x86/efi: Correct EFI boot stub use of code32_start
code32_start should point at the start of the protected mode code, and
*not* at the beginning of the bzImage. This is much easier to do in
assembly so document that callers of make_boot_params() need to fill out
code32_start.
The fallout from this bug is that we would end up relocating the image
but copying the image at some offset, resulting in what appeared to be
memory corruption.

x86/efi: Fix boot failure with EFI stub
commit
54b52d872680 ("x86/efi: Build our own EFI services pointer
table") introduced a regression because the 64-bit file_size()
implementation passed a pointer to a 32-bit data object, instead of a
pointer to a 64-bit object.
Because the firmware treats the object as 64-bits regardless it was
reading random values from the stack for the upper 32-bits.
This resulted in people being unable to boot their machines, after
seeing the following error messages,
Failed to get file info size
Failed to alloc highmem for files

x86/apic: Reinstate error IRQ Pentium erratum 3AP workaround
A change introduced with commit
60283df7ac26a4fe2d56631ca2946e04725e7eaf
("x86/apic: Read Error Status Register correctly") removed a read from the
APIC ESR register made before writing to same required to retrieve the
correct error status on Pentium systems affected by the 3AP erratum[1]:
"3AP. Writes to Error Register Clears Register
PROBLEM: The APIC Error register is intended to only be read.
If there is a write to this register the data in the APIC Error
register will be cleared and lost.
IMPLICATION: There is a possibility of clearing the Error
register status since the write to the register is not
specifically blocked.
WORKAROUND: Writes should not occur to the Pentium processor
APIC Error register.
STATUS: For the steppings affected see the Summary Table of
Changes at the beginning of this section."
The steppings affected are actually: B1, B3 and B5.
To avoid this information loss this change avoids the write to
ESR on all Pentium systems where it is actually never needed;
in Pentium processor documentation ESR was noted read-only and
the write only required for future architectural
compatibility[2].
The approach taken is the same as in lapic_setup_esr().

Pull s390 compat wrapper rework from Heiko Carstens:
"S390 compat system call wrapper simplification work.
The intention of this work is to get rid of all hand written assembly
compat system call wrappers on s390, which perform proper sign or zero
extension, or pointer conversion of compat system call parameters.
Instead all of this should be done with C code eg by using Al's
COMPAT_SYSCALL_DEFINEx() macro.
Therefore all common code and s390 specific compat system calls have
been converted to the COMPAT_SYSCALL_DEFINEx() macro.
In order to generate correct code all compat system calls may only
have eg compat_ulong_t parameters, but no unsigned long parameters.
Those patches which change parameter types from unsigned long to
compat_ulong_t parameters are separate in this series, but shouldn't
cause any harm.
The only compat system calls which intentionally have 64 bit
parameters (preadv64 and pwritev64) in support of the x86/32 ABI
haven't been changed, but are now only available if an architecture
defines __ARCH_WANT_COMPAT_SYS_PREADV64/PWRITEV64.
System calls which do not have a compat variant but still need proper
zero extension on s390, like eg "long sys_brk(unsigned long brk)" will
get a proper wrapper function with the new s390 specific
COMPAT_SYSCALL_WRAPx() macro:

which generates the following code (simplified):

Given that the C file which contains all the COMPAT_SYSCALL_WRAP lines
includes both linux/syscall.h and linux/compat.h, it will generate
build errors, if the declaration of sys_brk() doesn't match, or if
there exists a non-matching compat_sys_brk() declaration.
In addition this will intentionally result in a link error if
somewhere else a compat_sys_brk() function exists, which probably
should have been used instead. Two more BUILD_BUG_ONs make sure the
size and type of each compat syscall parameter can be handled
correctly with the s390 specific macros.
I converted the compat system calls step by step to verify the
generated code is correct and matches the previous code. In fact it
did not always match, however that was always a bug in the hand
written asm code.
In result we get less code, less bugs, and much more sanity checking"

x86: Adjust irq remapping quirk for older revisions of 5500/5520 chipsets
Commit
03bbcb2e7e2 (iommu/vt-d: add quirk for broken interrupt
remapping on 55XX chipsets) properly disables irq remapping on the
5500/5520 chipsets that don't correctly perform that feature.
However, when I wrote it, I followed the errata sheet linked in that
commit too closely, and explicitly tied the activation of the quirk to
revision 0x13 of the chip, under the assumption that earlier revisions
were not in the field. Recently a system was reported to be suffering
from this remap bug and the quirk hadn't triggered, because the
revision id register read at a lower value that 0x13, so the quirk
test failed improperly. Given this, it seems only prudent to adjust
this quirk so that any revision less than 0x13 has the quirk asserted.
[ tglx: Removed the 0x12 comparison of pci id 3405 as this is covered
by the <= 0x13 check already ]

irqchip: sun7i/sun6i: Disable NMI before registering the handler
It is advisable to disable the NMI before registering the IRQ handler as
registering the IRQ handler unmasks the IRQ on the GIC, so if U-Boot has
left the NMI enabled and the NMI pin is active we will immediately get
an interrupt before any driver has claimed the downstream interrupt of
the NMI.

ARM: sun7i/sun6i: dts: Fix IRQ number for sun6i NMI controller
The IRQ line used in sun6i-a31.dtsi for the NMI controller is wrong.
This causes a IRQ storm since the NMI controller is repeatedly fired.
This patch fixes this problem assigning the correct IRQ number to the
NMI controller.

avr32: remove cpu_data macro to fix compiles
Having cpu_data as a parameterless macro can easily cause build failures
because it can be a variable name like in linux/pm_domain.h [1]. So,
remove the macro and convert its only user. Because this architecture
cannot do SMP, remove the whole SMP block, too. Only compile tested due
to no hardware.

AUDIT: Allow login in non-init namespaces
It its possible to configure your PAM stack to refuse login if audit
messages (about the login) were unable to be sent. This is common in
many distros and thus normal configuration of many containers. The PAM
modules determine if audit is enabled/disabled in the kernel based on
the return value from sending an audit message on the netlink socket.
If userspace gets back ECONNREFUSED it believes audit is disabled in the
kernel. If it gets any other error else it refuses to let the login
proceed.
Just about ever since the introduction of namespaces the kernel audit
subsystem has returned EPERM if the task sending a message was not in
the init user or pid namespace. So many forms of containers have never
worked if audit was enabled in the kernel.
BUT if the container was not in net_init then the kernel network code
would send ECONNREFUSED (instead of the audit code sending EPERM). Thus
by pure accident/dumb luck/bug if an admin configured the PAM stack to
reject all logins that didn't talk to audit, but then ran the login
untility in the non-init_net namespace, it would work!! Clearly this was
a bug, but it is a bug some people expected.
With the introduction of network namespace support in 3.14-rc1 the two
bugs stopped cancelling each other out. Now, containers in the
non-init_net namespace refused to let users log in (just like PAM was
configfured!) Obviously some people were not happy that what used to let
users log in, now didn't!
This fix is kinda hacky. We return ECONNREFUSED for all non-init
relevant namespaces. That means that not only will the old broken
non-init_net setups continue to work, now the broken non-init_pid or
non-init_user setups will 'work'. They don't really work, since audit
isn't logging things. But it's what most users want.
In 3.15 we should have patches to support not only the non-init_net
(3.14) namespace but also the non-init_pid and non-init_user namespace.
So all will be right in the world. This just opens the doors wide open
on 3.14 and hopefully makes users happy, if not the audit system...

ext4: atomically set inode->i_flags in ext4_set_inode_flags()
Use cmpxchg() to atomically set i_flags instead of clearing out the
S_IMMUTABLE, S_APPEND, etc. flags and then setting them from the
EXT4_IMMUTABLE_FL, EXT4_APPEND_FL flags, since this opens up a race
where an immutable file has the immutable flag cleared for a brief
window of time.

fixes RCU bug - walking through hlist is safe in face of element moves,
since it's self-terminating. Cyclic lists are not - if we end up jumping
to another hash chain, we'll loop infinitely without ever hitting the
original list head.

there's no mounts to propagate to and thus no copies to create.

We should not be using static variable mousedev_mix in methods that can be
called before that singleton gets assigned. While at it let's add open and
close methods to mousedev structure so that we do not need to test if we
are dealing with multiplexor or normal device and simply call appropriate
method directly.

Input: don't modify the id of ioctl-provided ff effect on upload failure
If a new (id == -1) ff effect was uploaded from userspace,
ff-core.c::input_ff_upload() will have assigned a positive number to the
new effect id. Currently, evdev.c::evdev_do_ioctl() will save this new id
to userspace, regardless of whether the upload succeeded or not.
On upload failure, this can be confusing because the dev->ff->effects[]
array will not contain an element at the index of that new effect id.
This patch fixes this by leaving the id unchanged after upload fails.
Note: Unfortunately applications should still expect changed effect id for
quite some time.

rbd: drop an unsafe assertion
Olivier Bonvalet reported having repeated crashes due to a failed
assertion he was hitting in rbd_img_obj_callback():
Assertion failure in rbd_img_obj_callback() at line 2165:
rbd_assert(which >= img_request->next_completion);
With a lot of help from Olivier with reproducing the problem
we were able to determine the object and image requests had
already been completed (and often freed) at the point the
assertion failed.
There was a great deal of discussion on the ceph-devel mailing list
about this. The problem only arose when there were two (or more)
object requests in an image request, and the problem was always
seen when the second request was being completed.
The problem is due to a race in the window between setting the
"done" flag on an object request and checking the image request's
next completion value. When the first object request completes, it
checks to see if its successor request is marked "done", and if
so, that request is also completed. In the process, the image
request's next_completion value is updated to reflect that both
the first and second requests are completed. By the time the
second request is able to check the next_completion value, it
has been set to a value *greater* than its own "which" value,
which caused an assertion to fail.
Fix this problem by skipping over any completion processing
unless the completing object request is the next one expected.
Test only for inequality (not >=), and eliminate the bad
assertion.

percpu: renew the max_contig if we merge the head and previous block
During pcpu_alloc_area(), we might merge the current head with the
previous block. Since we have calculated the max_contig using the
size of previous block before we skip it, and now we update the size
of previous block, so we should renew the max_contig.

qlge: Do not propaged vlan tag offloads to vlans
qlge driver turns off NETIF_F_HW_CTAG_FILTER, but forgets to
turn off HW_CTAG_TX and HW_CTAG_RX on vlan devices. With the
current settings, q-in-q will only generate a single vlan header.
Remember to mask off CTAG_TX and CTAG_RX features in vlan_features.

bridge: Fix crash with vlan filtering and tcpdump
When the vlan filtering is enabled on the bridge, but
the filter is not configured on the bridge device itself,
running tcpdump on the bridge device will result in a
an Oops with NULL pointer dereference. The reason
is that br_pass_frame_up() will bypass the vlan
check because promisc flag is set. It will then try
to get the table pointer and process the packet based
on the table. Since the table pointer is NULL, we oops.
Catch this special condition in br_handle_vlan().

net: Account for all vlan headers in skb_mac_gso_segment
skb_network_protocol() already accounts for multiple vlan
headers that may be present in the skb. However, skb_mac_gso_segment()
doesn't know anything about it and assumes that skb->mac_len
is set correctly to skip all mac headers. That may not
always be the case. If we are simply forwarding the packet (via
bridge or macvtap), all vlan headers may not be accounted for.
A simple solution is to allow skb_network_protocol to return
the vlan depth it has calculated. This way skb_mac_gso_segment
will correctly skip all mac headers.

x86: fix boot on uniprocessor systems
On x86 uniprocessor systems topology_physical_package_id() returns -1
which causes rapl_cpu_prepare() to leave rapl_pmu variable uninitialized
which leads to GPF in rapl_pmu_init().
See arch/x86/kernel/cpu/perf_event_intel_rapl.c.
It turns out that physical_package_id and core_id can actually be
retreived for uniprocessor systems too. Enabling them also fixes
rapl_pmu code.

ipv6: move DAD and addrconf_verify processing to workqueue
addrconf_join_solict and addrconf_join_anycast may cause actions which
need rtnl locked, especially on first address creation.
A new DAD state is introduced which defers processing of the initial
DAD processing into a workqueue.
To get rtnl lock we need to push the code paths which depend on those
calls up to workqueues, specifically addrconf_verify and the DAD
processing.

addrconf_dad_failure needs to be queued up to the workqueue, too. This
patch introduces a new DAD state and stop the DAD processing in the
workqueue (this is because of the possible ipv6_del_addr processing
which removes the solicited multicast address from the device).
addrconf_verify_lock is removed, too. After the transition it is not
needed any more.
As we are not processing in bottom half anymore we need to be a bit more
careful about disabling bottom half out when we lock spin_locks which are also
used in bh.

openvswitch: fix a possible deadlock and lockdep warning
There are two problematic situations.
A deadlock can happen when is_percpu is false because it can get
interrupted while holding the spinlock. Then it executes
ovs_flow_stats_update() in softirq context which tries to get
the same lock.
The second sitation is that when is_percpu is true, the code
correctly disables BH but only for the local CPU, so the
following can happen when locking the remote CPU without
disabling BH:

The problem is that our logic that determines that the storm has
ended is incorrect. We announce the end, re-enable interrupts and
realize that the storm is still going on, so we switch back to
polling mode. Rinse, repeat.
When a storm happens we disable signaling of errors via CMCI and begin
polling machine check banks instead. If we find any logged errors,
then we need to set a per-cpu flag so that our per-cpu tests that
check whether the storm is ongoing will see that errors are still
being logged independently of whether mce_notify_irq() says that the
error has been fully processed.
cmci_clear() is not the right tool to disable a bank. It disables the
interrupt for the bank as desired, but it also clears the bit for
this bank in "mce_banks_owned" so we will skip the bank when polling
(so we fail to see that the storm continues because we stop looking).
New cmci_storm_disable_banks() just disables the interrupt while
allowing polling to continue.

bridge: Fix handling stacked vlan tags
If a bridge with vlan_filtering enabled receives frames with stacked
vlan tags, i.e., they have two vlan tags, br_vlan_untag() strips not
only the outer tag but also the inner tag.
br_vlan_untag() is called only from br_handle_vlan(), and in this case,
it is enough to set skb->vlan_tci to 0 here, because vlan_tci has already
been set before calling br_handle_vlan().

bridge: Fix inabillity to retrieve vlan tags when tx offload is disabled
Bridge vlan code (br_vlan_get_tag()) assumes that all frames have vlan_tci
if they are tagged, but if vlan tx offload is manually disabled on bridge
device and frames are sent from vlan device on the bridge device, the tags
are embedded in skb->data and they break this assumption.
Extract embedded vlan tags and move them to vlan_tci at ingress.

vhost: fix total length when packets are too short
When mergeable buffers are disabled, and the
incoming packet is too large for the rx buffer,
get_rx_bufs returns success.
This was intentional in order for make recvmsg
truncate the packet and then handle_rx would
detect err != sock_len and drop it.
Unfortunately we pass the original sock_len to
recvmsg - which means we use parts of iov not fully
validated.
Fix this up by detecting this overrun and doing packet drop
immediately.

random32: avoid attempt to late reseed if in the middle of seeding
Commit
4af712e8df ("random32: add prandom_reseed_late() and call when
nonblocking pool becomes initialized") has added a late reseed stage
that happens as soon as the nonblocking pool is marked as initialized.
This fails in the case that the nonblocking pool gets initialized
during __prandom_reseed()'s call to get_random_bytes(). In that case
we'd double back into __prandom_reseed() in an attempt to do a late
reseed - deadlocking on 'lock' early on in the boot process.
Instead, just avoid even waiting to do a reseed if a reseed is already
occuring.

random32: assign to network folks in MAINTAINERS
lib/random32.c was split out of the network code and is de-facto
still maintained by the almighty net/ gods.
Make it a bit more official so that people who aren't aware of
that know where to send their patches.

Input: synaptics - add manual min/max quirk
The new Lenovo Haswell series (-40's) contains a new Synaptics touchpad.
However, these new Synaptics devices report bad axis ranges.
Under Windows, it is not a problem because the Windows driver uses RMI4
over SMBus to talk to the device. Under Linux, we are using the PS/2
fallback interface and it occurs the reported ranges are wrong.
Of course, it would be too easy to have only one range for the whole
series, each touchpad seems to be calibrated in a different way.
We can not use SMBus to get the actual range because I suspect the firmware
will switch into the SMBus mode and stop talking through PS/2 (this is the
case for hybrid HID over I2C / PS/2 Synaptics touchpads).
So as a temporary solution (until RMI4 land into upstream), start a new
list of quirks with the min/max manually set.

time: Revert to calling clock_was_set_delayed() while in irq context
In commit
47a1b796306356f35 ("tick/timekeeping: Call
update_wall_time outside the jiffies lock"), we moved to calling
clock_was_set() due to the fact that we were no longer holding
the timekeeping or jiffies lock.
However, there is still the problem that clock_was_set()
triggers an IPI, which cannot be done from the timer's hard irq
context, and will generate WARN_ON warnings.
Apparently in my earlier testing, I'm guessing I didn't bump the
dmesg log level, so I somehow missed the WARN_ONs.
Thus we need to revert back to calling clock_was_set_delayed().

Same fix as for nouveau, when we fail with EINVAL, subsequent
gets fail hard, causing the device not to open.
