KVM: nVMX: fix "acknowledge interrupt on exit" when APICv is in use
After commit
77b0f5d (KVM: nVMX: Ack and write vector info to intr_info
if L1 asks us to), "Acknowledge interrupt on exit" behavior can be
emulated. To do so, KVM will ask the APIC for the interrupt vector if
during a nested vmexit if VM_EXIT_ACK_INTR_ON_EXIT is set. With APICv,
kvm_get_apic_interrupt would return -1 and give the following WARNING:

To fix this, we cannot rely on the processor's virtual interrupt delivery,
because "acknowledge interrupt on exit" must only update the virtual
ISR/PPR/IRR registers (and SVI, which is just a cache of the virtual ISR)
but it should not deliver the interrupt through the IDT. Thus, KVM has
to deliver the interrupt "by hand", similar to the treatment of EOI in
commit
fc57ac2c9ca8 (KVM: lapic: sync highest ISR to hardware apic on
EOI, 2014-05-14).
The patch modifies kvm_cpu_get_interrupt to always acknowledge an
interrupt; there are only two callers, and the other is not affected
because it is never reached with kvm_apic_vid_enabled() == true. Then it
modifies apic_set_isr and apic_clear_irr to update SVI and RVI in addition
to the registers.

KVM: nVMX: Fix nested vmexit ack intr before load vmcs01
An external interrupt will cause a vmexit with reason "external interrupt"
when L2 is running. L1 will pick up the interrupt through vmcs12 if
L1 set the ack interrupt bit. Commit
77b0f5d (KVM: nVMX: Ack and write
vector info to intr_info if L1 asks us to) retrieves the interrupt that
belongs to L1 before vmcs01 is loaded.
This will lead to problems in the next patch, which would write to SVI
of vmcs02 instead of vmcs01 (SVI of vmcs02 doesn't make sense because
L2 runs without APICv).

KVM: PPC: Enable IRQFD support for the XICS interrupt controller
This makes it possible to use IRQFDs on platforms that use the XICS
interrupt controller. To do this we implement kvm_irq_map_gsi() and
kvm_irq_map_chip_pin() in book3s_xics.c, so as to provide a 1-1 mapping
between global interrupt numbers and XICS interrupt source numbers.
For now, all interrupts are mapped as "IRQCHIP" interrupts, and no
MSI support is provided.
This means that kvm_set_irq can now get called with level == 0 or 1
as well as the powerpc-specific values KVM_INTERRUPT_SET,
KVM_INTERRUPT_UNSET and KVM_INTERRUPT_SET_LEVEL. We change
ics_deliver_irq() to accept all those values, and remove its
report_status argument, as it is always false, given that we don't
support KVM_IRQ_LINE_STATUS.
This also adds support for interrupt ack notifiers to the XICS code
so that the IRQFD resampler functionality can be supported.

KVM: Give IRQFD its own separate enabling Kconfig option
Currently, the IRQFD code is conditional on CONFIG_HAVE_KVM_IRQ_ROUTING.
So that we can have the IRQFD code compiled in without having the
IRQ routing code, this creates a new CONFIG_HAVE_KVM_IRQFD, makes
the IRQFD code conditional on it instead of CONFIG_HAVE_KVM_IRQ_ROUTING,
and makes all the platforms that currently select HAVE_KVM_IRQ_ROUTING
also select HAVE_KVM_IRQFD.

KVM: Move irq notifier implementation into eventfd.c
This moves the functions kvm_irq_has_notifier(), kvm_notify_acked_irq(),
kvm_register_irq_ack_notifier() and kvm_unregister_irq_ack_notifier()
from irqchip.c to eventfd.c. The reason for doing this is that those
functions are used in connection with IRQFDs, which are implemented in
eventfd.c. In future we will want to use IRQFDs on platforms that
don't implement the GSI routing implemented in irqchip.c, so we won't
be compiling in irqchip.c, but we still need the irq notifiers. The
implementation is unchanged.

KVM: Move all accesses to kvm::irq_routing into irqchip.c
Now that struct _irqfd does not keep a reference to storage pointed
to by the irq_routing field of struct kvm, we can move the statement
that updates it out from under the irqfds.lock and put it in
kvm_set_irq_routing() instead. That means we then have to take a
srcu_read_lock on kvm->irq_srcu around the irqfd_update call in
kvm_irqfd_assign(), since holding the kvm->irqfds.lock no longer
ensures that that the routing can't change.
Combined with changing kvm_irq_map_gsi() and kvm_irq_map_chip_pin()
to take a struct kvm * argument instead of the pointer to the routing
table, this allows us to to move all references to kvm->irq_routing
into irqchip.c. That in turn allows us to move the definition of the
kvm_irq_routing_table struct into irqchip.c as well.

KVM: irqchip: Provide and use accessors for irq routing table
This provides accessor functions for the KVM interrupt mappings, in
order to reduce the amount of code that accesses the fields of the
kvm_irq_routing_table struct, and restrict that code to one file,
virt/kvm/irqchip.c. The new functions are kvm_irq_map_gsi(), which
maps from a global interrupt number to a set of IRQ routing entries,
and kvm_irq_map_chip_pin, which maps from IRQ chip and pin numbers to
a global interrupt number.
This also moves the update of kvm_irq_routing_table::chip[][]
into irqchip.c, out of the various kvm_set_routing_entry
implementations. That means that none of the kvm_set_routing_entry
implementations need the kvm_irq_routing_table argument anymore,
so this removes it.
This does not change any locking or data lifetime rules.

KVM: Don't keep reference to irq routing table in irqfd struct
This makes the irqfd code keep a copy of the irq routing table entry
for each irqfd, rather than a reference to the copy in the actual
irq routing table maintained in kvm/virt/irqchip.c. This will enable
us to change the routing table structure in future, or even not have a
routing table at all on some platforms.
The synchronization that was previously achieved using srcu_dereference
on the read side is now achieved using a seqcount_t structure. That
ensures that we don't get a halfway-updated copy of the structure if
we read it while another thread is updating it.
We still use srcu_read_lock/unlock around the read side so that when
changing the routing table we can be sure that after calling
synchronize_srcu, nothing will be using the old routing.

caused a build failure with allyesconfig:
arch/powerpc/kvm/kvm-pr.o:(__tracepoints+0xa8): multiple definition of `__tracepoint_kvm_ppc_instr'
arch/powerpc/kvm/kvm.o:(__tracepoints+0x1c0): first defined here
due to a duplicate definition of the tracepoint in trace.h and
trace_pr.h. Because the tracepoint is still used by Book3S HV
code, and because the PR code does include trace.h, just remove
the duplicate definition from trace_pr.h, and export it from
kvm.o.

"This pull request has a lot of work done. The main thing is the
changes to the ftrace function callback infrastructure. It's
introducing a way to allow different functions to call directly
different trampolines instead of all calling the same "mcount" one.
The only user of this for now is the function graph tracer, which
always had a different trampoline, but the function tracer trampoline
was called and did basically nothing, and then the function graph
tracer trampoline was called. The difference now, is that the
function graph tracer trampoline can be called directly if a function
is only being traced by the function graph trampoline. If function
tracing is also happening on the same function, the old way is still
done.
The accounting for this takes up more memory when function graph
tracing is activated, as it needs to keep track of which functions it
uses. I have a new way that wont take as much memory, but it's not
ready yet for this merge window, and will have to wait for the next
one.
Another big change was the removal of the ftrace_start/stop() calls
that were used by the suspend/resume code that stopped function
tracing when entering into suspend and resume paths. The stop of
ftrace was done because there was some function that would crash the
system if one called smp_processor_id()! The stop/start was a big
hammer to solve the issue at the time, which was when ftrace was first
introduced into Linux. Now ftrace has better infrastructure to debug
such issues, and I found the problem function and labeled it with
"notrace" and function tracing can now safely be activated all the way
down into the guts of suspend and resume

trace_seq() code, and other various small fixes and clean ups to
ftrace and tracing"

the similar style with other controllers so that an ancestor's
configuration change doesn't change the descendants' configurations

ARM: 8124/1: don't enter kgdb when userspace executes a kgdb break instruction
The kgdb breakpoint hooks (kgdb_brk_fn and kgdb_compiled_brk_fn)
should only be entered when a kgdb break instruction is executed
from the kernel. Otherwise, if kgdb is enabled, a userspace program
can cause the kernel to drop into the debugger by executing either
KGDB_BREAKINST or KGDB_COMPILED_BREAK.

ARM: idmap: add identity mapping usage note
Add a note about the usage of the identity mapping; we do not support
accesses outside of the identity map region and kernel image while a
CPU is using the identity map. This is because the identity mapping
may overwrite vmalloc space, IO mappings, the vectors pages, etc.

dm cache: fix race affecting dirty block count
nr_dirty is updated without locking, causing it to drift so that it is
non-zero (either a small positive integer, or a very large one when an
underflow occurs) even when there are no actual dirty blocks. This was
due to a race between the workqueue and map function accessing nr_dirty
in parallel without proper protection.
People were seeing under runs due to a race on increment/decrement of
nr_dirty, see: https://lkml.org/lkml/2014/6/3/648
Fix this by using an atomic_t for nr_dirty.

dm bufio: fully initialize shrinker
1d3d4437eae1 ("vmscan: per-node deferred work") added a flags field to
struct shrinker assuming that all shrinkers were zero filled. The dm
bufio shrinker is not zero filled, which leaves arbitrary kmalloc() data
in flags. So far the only defined flags bit is SHRINKER_NUMA_AWARE.
But there are proposed patches which add other bits to shrinker.flags
(e.g. memcg awareness).
Rather than simply initializing the shrinker, this patch uses kzalloc()
when allocating the dm_bufio_client to ensure that the embedded shrinker
and any other similar structures are zeroed.
This fixes theoretical over aggressive shrinking of dm bufio objects.
If the uninitialized dm_bufio_client.shrinker.flags contains
SHRINKER_NUMA_AWARE then shrink_slab() would call the dm shrinker for
each numa node rather than just once. This has been broken since 3.12.

ata: libahci: Silence compiler warning on 64-bit

725c7b570fda (ata: libahci_platform: move port_map parameters
into the AHCI structure) moves flags into the struct ahci_host_priv's
.flags field, which causes compiler warnings on 64-bit builds when that
value is cast to a void * pointer. Cast to an unsigned long so that the
subsequent cast to a pointer doesn't produce a warning.

crypto: drbg - fix failure of generating multiple of 2**16 bytes
The function drbg_generate_long slices the request into 2**16 byte
or smaller chunks. However, the loop, however invokes the random number
generation function with zero bytes when the request size is a multiple
of 2**16 bytes. The fix prevents zero bytes requests.

The CCP hardware interprets all numbers as unsigned numbers, therefore
sign extending input data is not valid. Modify the function calls
for RSA and ECC to not perform sign extending.

arm64: KVM: fix 64bit CP15 VM access for 32bit guests
Commit
f0a3eaff71b8 (ARM64: KVM: fix big endian issue in
access_vm_reg for 32bit guest) changed the way we handle CP15
VM accesses, so that all 64bit accesses are done via vcpu_sys_reg.
This looks like a good idea as it solves indianness issues in an
elegant way, except for one small detail: the register index is
doesn't refer to the same array! We end up corrupting some random
data structure instead.
Fix this by reverting to the original code, except for the introduction
of a vcpu_cp15_64_high macro that deals with the endianness thing.
Tested on Juno with 32bit SMP guests.

direct-io: fix AIO regression
The direct-io.c rewrite to use the iov_iter infrastructure stopped updating
the size field in struct dio_submit, and thus rendered the check for
allowing asynchronous completions to always return false. Fix this by
comparing it to the count of bytes in the iov_iter instead.

x86/kvm: Resolve shadow warnings in macro expansion
Resolve shadow warnings that appear in W=2 builds. Instead of
using ret to hold the return pointer, save the length in a new
variable saved_len and compute the pointer on exit. This also
resolves a very technical error, in that ret was declared as
a const char *, when it really was a char * const.

KVM: arm64: GICv3: mandate page-aligned GICV region
Just like GICv2 was fixed in
63afbe7a0ac1
(kvm: arm64: vgic: fix hyp panic with 64k pages on juno platform),
mandate the GICV region to be both aligned on a page boundary and
its size to be a multiple of page size.
This prevents a guest from being able to poke at regions where we
have no idea what is sitting there.

crypto: af_alg - properly label AF_ALG socket
Th AF_ALG socket was missing a security label (e.g. SELinux)
which means that socket was in "unlabeled" state.
This was recently demonstrated in the cryptsetup package
(cryptsetup v1.6.5 and later.)

This patch clones the sock's label from the parent sock

arm64: KVM: GICv3: move system register access to msr_s/mrs_s
Commit
72c583951526 (arm64: gicv3: Allow GICv3 compilation with
older binutils) changed the way we express the GICv3 system registers,
but couldn't change the occurences used by KVM as the code wasn't
merged yet.
Just fix the accessors.

arm64: don't call break hooks for BRK exceptions from EL0
Our break hooks are used to handle brk exceptions from kgdb (and potentially
kprobes if that code ever resurfaces), so don't bother calling them if
the BRK exception comes from userspace.
This prevents userspace from trapping to a kdb shell on systems where
kgdb is enabled and active.

KVM: PPC: PR: Handle FSCR feature deselects
We handle FSCR feature bits (well, TAR only really today) lazily when the guest
starts using them. So when a guest activates the bit and later uses that feature
we enable it for real in hardware.
However, when the guest stops using that bit we don't stop setting it in
hardware. That means we can potentially lose a trap that the guest expects to
happen because it thinks a feature is not active.
This patch adds support to drop TAR when then guest turns it off in FSCR. While
at it it also restricts FSCR access to 64bit systems - 32bit ones don't have it.

KVM: s390: rework broken SIGP STOP interrupt handling
A VCPU might never stop if it intercepts (for whatever reason) between
"fake interrupt delivery" and execution of the stop function.
Heart of the problem is that SIGP STOP is an interrupt that has to be
processed on every SIE entry until the VCPU finally executes the stop
function.
This problem was made apparent by commit
7dfc63cf977447e09b1072911c2
(KVM: s390: allow only one SIGP STOP (AND STORE STATUS) at a time).
With the old code, the guest could (incorrectly) inject SIGP STOPs
multiple times. The bug of losing a sigp stop exists in KVM before
7dfc63cf97, but it was hidden by Linux guests doing a sigp stop loop.
The new code (rightfully) returns CC=2 and does not queue a new
interrupt.
This patch is a simple fix of the problem. Longterm we are going to
rework that code - e.g. get rid of the action bits and so on.

kexec: export free_huge_page to VMCOREINFO
PG_head_mask was added into VMCOREINFO to filter huge pages in
b3acc56bfe1
("kexec: save PG_head_mask in VMCOREINFO"), but makedumpfile still need
another symbol to filter *hugetlbfs* pages.
If a user hope to filter user pages, makedumpfile tries to exclude them by
checking the condition whether the page is anonymous, but hugetlbfs pages
aren't anonymous while they also be user pages.
We know it's possible to detect them in the same way as PageHuge(),
so we need the start address of free_huge_page():

mm: debugfs: move rounddown_pow_of_two() out from do_fault path
do_fault_around() expects fault_around_bytes rounded down to nearest page
order. Instead of calling rounddown_pow_of_two every time in
fault_around_pages()/fault_around_mask() we could do round down when user
changes fault_around_bytes via debugfs interface.
This also fixes bug when user set fault_around_bytes to 0. Result of
rounddown_pow_of_two(0) is not defined, therefore fault_around_bytes == 0
doesn't work without this patch.
Let's set fault_around_bytes to PAGE_SIZE if user sets to something less
than PAGE_SIZE

Commit
fb2a6fc56be6 ("mm: memcg: rework and document OOM waiting and
wakeup") has moved mem_cgroup_oom_notify outside of memcg_oom_lock
assuming it is protected by the hierarchical OOM-lock.
Although this is true for the notification part the protection doesn't
cover unregistration of event which can happen in parallel now so
mem_cgroup_oom_notify can see already unlinked and/or freed
mem_cgroup_eventfd_list.
Fix this by using memcg_oom_lock also in mem_cgroup_oom_notify.

hwpoison: call action_result() in failure path of hwpoison_user_mappings()
hwpoison_user_mappings() could fail for various reasons, so printk()s to
print out the reasons should be done in each failure check inside
hwpoison_user_mappings().
And currently we don't call action_result() when hwpoison_user_mappings()
fails, which is not consistent with other exit points of memory error
handler. So this patch fixes these messaging problems.

hwpoison: fix hugetlbfs/thp precheck in hwpoison_user_mappings()
A recent fix from Chen Yucong, commit
0bc1f8b0682c ("hwpoison: fix the
handling path of the victimized page frame that belong to non-LRU")
rejects going into unmapping operation for hugetlbfs/thp pages, which
results in failing error containing on such pages. This patch fixes it.
With this patch, hwpoison functional tests in mce-test testsuite pass.

rapidio/tsi721_dma: fix failure to obtain transaction descriptor
This is a bug fix for the situation when function tsi721_desc_get() fails
to obtain a free transaction descriptor.
The bug usually results in a memory access crash dump when data transfer
scatter-gather list has more entries than size of hardware buffer
descriptors ring. This fix ensures that error is properly returned to a
caller instead of an invalid entry.
This patch is applicable to kernel versions starting from v3.5.

mm, thp: do not allow thp faults to avoid cpuset restrictions
The page allocator relies on __GFP_WAIT to determine if ALLOC_CPUSET
should be set in allocflags. ALLOC_CPUSET controls if a page allocation
should be restricted only to the set of allowed cpuset mems.
Transparent hugepages clears __GFP_WAIT when defrag is disabled to prevent
the fault path from using memory compaction or direct reclaim. Thus, it
is unfairly able to allocate outside of its cpuset mems restriction as a
side-effect.
This patch ensures that ALLOC_CPUSET is only cleared when the gfp mask is
truly GFP_ATOMIC by verifying it is also not a thp allocation.

mm/page-writeback.c: fix divide by zero in bdi_dirty_limits()
Under memory pressure, it is possible for dirty_thresh, calculated by
global_dirty_limits() in balance_dirty_pages(), to equal zero. Then, if
strictlimit is true, bdi_dirty_limits() tries to resolve the proportion:
bdi_bg_thresh : bdi_thresh = background_thresh : dirty_thresh
by dividing by zero.

ARM: nomadik: fix up double inversion in DT
The GPIO pin connected to card detect was inverted twice: once by
the argument to the GPIO line itself where it was magically marked
as active low by the flag GPIO_ACTIVE_LOW (0x01) in the third cell,
and also marked active low AGAIN by explicitly stating
"cd-inverted" (a deprecated method).
After commit
78f87df2b4f8760954d7d80603d0cfcbd4759683
"mmc: mmci: Use the common mmc DT parser" this results in the
line being inverted twice so it was effectively uninverted, while
the old code would not have this effect, instead disregarding the
flag on the GPIO line altogether, which is a bug. I admit the
semantics may be unclear but inverting twice is as good a
definition as any on how this should work.
So fix up the buggy device tree. Use proper #includes so the DTS
is clear and readable.

ata: libahci: allow to use multiple PHYs
The current implementation of the libahci does not allow to use multiple
PHYs. This patch adds the support of multiple PHYs by the libahci while
keeping the old bindings valid for device tree compatibility.
This introduce a new way of defining SATA ports in the device tree, with
one port per sub-node. This as the advantage of allowing a per port
configuration. Because some ports may be accessible but disabled in the
device tree, the port_map mask is computed automatically when using
this.

ata: libahci_platform: move port_map parameters into the AHCI structure
This patch moves force_port_map and mask_port_map into the
ahci_host_priv structure. This allows to modify them into the AHCI
framework. This is needed by the new dt bindings representing ports as
the port_map mask is computed automatically.
Parameters modifying force_port_map, mask_port_map and flags have been
removed from the ahci_platform_init_host() function, and inputs in the
ahci_host_priv structure are now directly filed.

KVM: x86: always exit on EOIs for interrupts listed in the IOAPIC redir table
Currently, the EOI exit bitmap (used for APICv) does not include
interrupts that are masked. However, this can cause a bug that manifests
as an interrupt storm inside the guest. Alex Williamson reported the
bug and is the one who really debugged this; I only wrote the patch. :)
The scenario involves a multi-function PCI device with OHCI and EHCI
USB functions and an audio function, all assigned to the guest, where
both USB functions use legacy INTx interrupts.
As soon as the guest boots, interrupts for these devices turn into an
interrupt storm in the guest; the host does not see the interrupt storm.
Basically the EOI path does not work, and the guest continues to see the
interrupt over and over, even after it attempts to mask it at the APIC.
The bug is only visible with older kernels (RHEL6.5, based on 2.6.32
with not many changes in the area of APIC/IOAPIC handling).
Alex then tried forcing bit 59 (corresponding to the USB functions' IRQ)
on in the eoi_exit_bitmap and TMR, and things then work. What happens
is that VFIO asserts IRQ11, then KVM recomputes the EOI exit bitmap.
It does not have set bit 59 because the RTE was masked, so the IOAPIC
never sees the EOI and the interrupt continues to fire in the guest.
My guess was that the guest is masking the interrupt in the redirection
table in the interrupt routine, i.e. while the interrupt is set in a
LAPIC's ISR, The simplest fix is to ignore the masking state, we would
rather have an unnecessary exit rather than a missed IRQ ACK and anyway
IOAPIC interrupts are not as performance-sensitive as for example MSIs.
Alex tested this patch and it fixed his bug.

"Device tree Exynos bug fix for v3.16-rc7
This bug fix has been brewing for a while. I hate sending it to you
so late, but I only got confirmation that it solves the problem this
past weekend. The diff looks big for a bug fix, but the majority of
it is only executed in the Exynos quirk case. Unfortunately it
required splitting early_init_dt_scan() in two and adding quirk
handling in the middle of it on ARM.
Exynos has buggy firmware that puts bad data into the memory node.
Commit
1c2f87c22566 ("ARM: Get rid of meminfo") exposed the bug by
dropping the artificial upper bound on the number of memory banks that
can be added. Exynos fails to boot after that commit. This branch
fixes it by splitting the early DT parse function and inserting a
fixup hook. Exynos uses the hook to correct the DT before parsing
memory regions"
