kdb: Remove stack dump when entering kgdb due to NMI
Issuing a stack dump feels ergonomically wrong when entering due to NMI.
Entering due to NMI is normally a reaction to a user request, either the
NMI button on a server or a "magic knock" on a UART. Therefore the
backtrace behaviour on entry due to NMI should be like SysRq-g (no stack
dump) rather than like oops.
Note also that the stack dump does not offer any information that
cannot be trivial retrieved using the 'bt' command.

kdb: Avoid printing KERN_ levels to consoles
Currently when kdb traps printk messages then the raw log level prefix
(consisting of '\001' followed by a numeral) does not get stripped off
before the message is issued to the various I/O handlers supported by
kdb. This causes annoying visual noise as well as causing problems
grepping for ^. It is also a change of behaviour compared to normal usage
of printk() usage. For example <SysRq>-h ends up with different output to
that of kdb's "sr h".
This patch addresses the problem by stripping log levels from messages
before they are issued to the I/O handlers. printk() which can also
act as an i/o handler in some cases is special cased; if the caller
provided a log level then the prefix will be preserved when sent to
printk().
The addition of non-printable characters to the output of kdb commands is a
regression, albeit and extremely elderly one, introduced by commit
04d2c8c83d0e ("printk: convert the format for KERN_<LEVEL> to a 2 byte
pattern"). Note also that this patch does *not* restore the original
behaviour from v3.5. Instead it makes printk() from within a kdb command
display the message without any prefix (i.e. like printk() normally does).

kdb: Fix off by one error in kdb_cpu()

* Fix an off-by-one error in kdb_cpu().

(kgdb-next contains a patch which introduced pr_fmt() to this file
to the tag will now be applied automatically).

kdb: fix incorrect counts in KDB summary command output
The output of KDB 'summary' command should report MemTotal, MemFree
and Buffers output in kB. Current codes report in unit of pages.
A define of K(x) as
is defined in the code, but not used.
This patch would apply the define to convert the values to kB.

"During testing Sedat Dilek hit a "suspicious RCU usage" splat that
pointed out a real bug. During suspend and resume the tlb_flush
tracepoint is called when the CPU is going offline. As the CPU has
been noted as offline, RCU is ignoring that CPU, which means that it
can not use RCU protected locks. When tracepoints are activated, they
require RCU locking, and if RCU is ignoring a CPU that runs a
tracepoint, there is a chance that the tracepoint could cause
corruption.
The solution was to change the tracepoint into a
TRACE_EVENT_CONDITION() which allows us to check a condition to
determine if the tracepoint should be called or not. If the condition
is not met, the rcu protected code will not be executed. By adding
the condition "cpu_online(smp_processor_id())", this will prevent the
RCU protected code from being executed if the CPU is marked offline.
After adding this, another bug was discovered. As RCU checks rcu
callers, if a rcu call is not done, there is no check (obviously). We
found that tracepoints could be added in RCU ignored locations and not
have lockdep complain until the tracepoint is activated. This missed
places where tracepoints were added in places they should not have
been. To fix this, code was added in 3.18 that if lockdep is enabled,
any tracepoint will still call the rcu checks even if the tracepoint
is not enabled. The bug here, is that the check does not take the
CONDITION into account. As the condition may prevent tracepoints from
being activated in RCU ignored areas (as the one patch does), we get
false positives when we enable lockdep and hit a tracepoint that the
condition prevents it from being called in a RCU ignored location.
The fix for this is to add the CONDITION to the rcu checks, even if
the tracepoint is not enabled"

x86/tlb/trace: Do not trace on CPU that is offline
When taking a CPU down for suspend and resume, a tracepoint may be called
when the CPU has been designated offline. As tracepoints require RCU for
protection, they must not be called if the current CPU is offline.
Unfortunately, trace_tlb_flush() is called in this scenario as was noted

By converting the tlb_flush tracepoint to a TRACE_EVENT_CONDITION where the

tracing: Add condition check to RCU lockdep checks
The trace_tlb_flush() tracepoint can be called when a CPU is going offline.
When a CPU is offline, RCU is no longer watching that CPU and since the
tracepoint is protected by RCU, it must not be called. To prevent the
tlb_flush tracepoint from being called when the CPU is offline, it was
converted to a TRACE_EVENT_CONDITION where the condition checks if the
CPU is online before calling the tracepoint.
Unfortunately, this was not enough to stop lockdep from complaining about
it. Even though the RCU protected code of the tracepoint will never be
called, the condition is hidden within the tracepoint, and even though the
condition prevents RCU code from being called, the lockdep checks are
outside the tracepoint (this is to test tracepoints even when they are not
enabled).
Even though tracepoints should be checked to be RCU safe when they are not
enabled, the condition should still be considered when checking RCU.

While commit
7e36ef8205ff ("IB/core: Temporarily disable
ex_query_device uverb") is correct as it makes the extended
QUERY_DEVICE uverb (which came as part of commit
5a77abf9a97a
("IB/core: Add support for extended query device caps") and commit
860f10a799c8 ("IB/core: Add flags for on demand paging support")) not
available to userspace, it doesn't address the initial issue regarding
ib_copy_to_udata() [1][2].
Additionally, further discussions around this new uverb seems to
conclude it would require a different data structure than the one
currently described in <rdma/ib_user_verbs.h> [3].
Both of these issues require a revert of the changes, so this patch
partially reverts commit

nilfs2: fix deadlock of segment constructor over I_SYNC flag
Nilfs2 eventually hangs in a stress test with fsstress program. This
issue was caused by the following deadlock over I_SYNC flag between
nilfs_segctor_thread() and writeback_sb_inodes():

writeback_sb_inodes() calls do_writepages() for dirty inodes after
setting I_SYNC flag on inode->i_state. do_writepages() in turn calls
nilfs_writepages(), which can run segment constructor and wait for its
completion. On the other hand, segment constructor calls iput(), which
can call evict() and wait for the I_SYNC flag on
inode_wait_for_writeback().
Since segment constructor doesn't know when I_SYNC will be set, it
cannot know whether iput() will block or not unless inode->i_nlink has a
non-zero count. We can prevent evict() from being called in iput() by
implementing sop->drop_inode(), but it's not preferable to leave inodes
with i_nlink == 0 for long periods because it even defers file
truncation and inode deallocation. So, this instead resolves the
deadlock by calling iput() asynchronously with a workqueue for inodes
with i_nlink == 0.

memcg, shmem: fix shmem migration to use lrucare
It has been reported that 965GM might trigger
VM_BUG_ON_PAGE(!lrucare && PageLRU(oldpage), oldpage)
in mem_cgroup_migrate when shmem wants to replace a swap cache page
because of shmem_should_replace_page (the page is allocated from an
inappropriate zone). shmem_replace_page expects that the oldpage is not
on LRU list and calls mem_cgroup_migrate without lrucare. This is
obviously incorrect because swapcache pages might be on the LRU list
(e.g. swapin readahead page).
Fix this by enabling lrucare for the migration in shmem_replace_page.
Also clarify that lrucare should be used even if one of the pages might
be on LRU list.
The BUG_ON will trigger only when CONFIG_DEBUG_VM is enabled but even
without that the migration code might leave the old page on an
inappropriate memcg' LRU which is not that critical because the page
would get removed with its last reference but it is still confusing.

The symbol 'high_memory' is provided on both MMU- and NOMMU-kernels, but
only one of them is exported, which leads to module build errors in
drivers that work fine built-in:

mm: pagewalk: call pte_hole() for VM_PFNMAP during walk_page_range
walk_page_range() silently skips vma having VM_PFNMAP set, which leads
to undesirable behaviour at client end (who called walk_page_range).
Userspace applications get the wrong data, so the effect is like just
confusing users (if the applications just display the data) or sometimes
killing the processes (if the applications do something with
misunderstanding virtual addresses due to the wrong data.)
For example for pagemap_read, when no callbacks are called against
VM_PFNMAP vma, pagemap_read may prepare pagemap data for next virtual
address range at wrong index.
Eventually userspace may get wrong pagemap data for a task.
Corresponding to a VM_PFNMAP marked vma region, kernel may report
mappings from subsequent vma regions. User space in turn may account
more pages (than really are) to the task.
In my case I was using procmem, procrack (Android utility) which uses
pagemap interface to account RSS pages of a task. Due to this bug it
was giving a wrong picture for vmas (with VM_PFNMAP set).

All sst firmware is provided under the intel directory of the linux-firmware
tree. By default this directory structure is kept when installing on a target
system. Change the path to expect a default linux-firmware installation.

IRQCHIP: mips-gic: Avoid rerouting timer IRQs for smp-cmp
Commit
e9de688dac65 ("irqchip: mips-gic: Support local interrupts")
changed the GIC irqchip driver so that all local interrupts were routed
to the same CPU pin used for external interrupts. Unfortunately this
causes a regression when smp-cmp is used. The CPUs are started by the
bootloader and put in a timer based waiting poll loop, but when their
timer interrupts are rerouted to a different IRQ pin which is not
unmasked they never wake up.
Since smp-cmp support is deprecated and everybody who was using it
should be switching to smp-cps which brings up the secondary CPUs
without bootloader assistance, I've gone for the simple fix which can be
easily removed once smp-cmp is removed, rather than a fully generic fix.
In __gic_init() the local GIC_VPE_TIMER_MAP register is read to find the
boot-time routing of the local timer interrupt, and a chained handler is
added to that CPU pin as well as the normal one.

hrtimer: Fix incorrect tai offset calculation for non high-res timer systems
I noticed some CLOCK_TAI timer test failures on one of my
less-frequently used configurations. And after digging in I
found in
76f4108892d9 (Cleanup hrtimer accessors to the
timekepeing state), the hrtimer_get_softirq_time tai offset
calucation was incorrectly rewritten, as the tai offset we
return shold be from CLOCK_MONOTONIC, and not CLOCK_REALTIME.
This results in CLOCK_TAI timers expiring early on non-highres
capable machines.
This patch fixes the issue, calculating the tai time properly
from the monotonic base.

cls_api.c: Fix dumping of non-existing actions' stats.
In tcf_exts_dump_stats(), ensure that exts->actions is not empty before
accessing the first element of that list and calling tcf_action_copy_stats()
on it. This fixes some random segvs when adding filters of type "basic" with
no particular action.
This also fixes the dumping of those "no-action" filters, which more often
than not made calls to tcf_action_copy_stats() fail and consequently netlink
attributes added by the caller to be removed by a call to nla_nest_cancel().

pkt_sched: fq: avoid hang when quantum 0
Configuring fq with quantum 0 hangs the system, presumably because of a
non-interruptible infinite loop. Either way quantum 0 does not make sense.
Reproduce with:

ping 127.0.0.1

drm/cirrus: Limit modes depending on bpp option
The commit [
8975626ea35a: drm/cirrus: allow 32bpp framebuffers for
cirrus drm] broke X modesetting driver because cirrus driver still
provides the full list of modes up to 1280x1024 while the 32bpp can
support only up to 800x600.
We might be able to filter out the invalid modes in mode_valid
callback, but unfortunately the bpp in question can't be referred
there for now (let me know if there is a better way to retrieve the
bpp for the probed fb).
So, instead, this patch adds the bpp module option to specify the
maximal bpp explicitly and limits the resolutions in get_modes
depending on its value.
The default value is set to 24 so that the existing stuff keeps
working. If you need a new 32bpp feature, specify cirrus.bpp=32
option explicitly.

net: rds: use correct size for max unacked packets and bytes
Max unacked packets/bytes is an int while sizeof(long) was used in the
sysctl table.
This means that when they were getting read we'd also leak kernel memory
to userspace along with the timeout values.

vhost/net: fix up num_buffers endian-ness
In virtio 1.0 mode, when mergeable buffers are enabled on a big-endian
host, num_buffers wasn't byte-swapped correctly, so large incoming
packets got corrupted.
To fix, fill it in within hdr - this also makes sure it gets
the correct type.

Turned off UFO support to virtio-net based devices due to issues
with IPv6 fragment id generation for UFO packets. The issue
was that IPv6 UFO/GSO implementation expects the fragment id
to be supplied in skb_shinfo(). However, for packets generated
by the VMs, the fragment id is not supplied which causes all
IPv6 fragments to have the id of 0.
The problem is that turning off UFO support on tap/macvtap
as well as virtio devices caused issues with migrations.
Migrations would fail when moving a vm from a kernel supporting
expecting UFO to work to the newer kernels that disabled UFO.
This series provides a partial solution to address the migration
issue. The series allows us to track whether skb_shinfo()->ip6_frag_id
has been set by treating value of 0 as unset.
This lets GSO code to generate fragment ids if they are necessary
(ex: packet was generated by VM or packet socket).
Since v3:
- Resolved build issue when IPv6 is a module.
- Removed trailing white space.

- Rebase and rebuild to make sure everything works. No changes
to the patches were done.

ipv6: Select fragment id during UFO segmentation if not set.
If the IPv6 fragment id has not been set and we perform
fragmentation due to UFO, select a new fragment id.
We now consider a fragment id of 0 as unset and if id selection
process returns 0 (after all the pertrubations), we set it to
0x80000000, thus giving us ample space not to create collisions
with the next packet we may have to fragment.
When doing UFO integrity checking, we also select the
fragment id if it has not be set yet. This is stored into
the skb_shinfo() thus allowing UFO to function correclty.
This patch also removes duplicate fragment id generation code
and moves ipv6_select_ident() into the header as it may be
used during GSO.

sched/deadline: Fix deadline parameter modification handling
Commit
67dfa1b756f2 ("sched/deadline: Implement cancel_dl_timer() to
use in switched_from_dl()") removed the hrtimer_try_cancel() function
call out from init_dl_task_timer(), which gets called from
__setparam_dl().
The result is that we can now re-init the timer while its active --
this is bad and corrupts timer state.
Furthermore; changing the parameters of an active deadline task is
tricky in that you want to maintain guarantees, while immediately
effective change would allow one to circumvent the CBS guarantees --
this too is bad, as one (bad) task should not be able to affect the
others.
Rework things to avoid both problems. We only need to initialize the
timer once, so move that to __sched_fork() for new tasks.
Then make sure __setparam_dl() doesn't affect the current running
state but only updates the parameters used to calculate the next
scheduling period -- this guarantees the CBS functions as expected
(albeit slightly pessimistic).
This however means we need to make sure __dl_clear_params() needs to
reset the active state otherwise new (and tasks flipping between
classes) will not properly (re)compute their first instance.

PCI: Handle read-only BARs on AMD CS553x devices
Some AMD CS553x devices have read-only BARs because of a firmware or
hardware defect. There's a workaround in quirk_cs5536_vsa(), but it no
longer works after
36e8164882ca ("PCI: Restore detection of read-only
BARs"). Prior to
36e8164882ca, we filled in res->start; afterwards we
leave it zeroed out. The quirk only updated the size, so the driver tried
to use a region starting at zero, which didn't work.
Expand quirk_cs5536_vsa() to read the base addresses from the BARs and
hard-code the sizes.
On Nix's system BAR 2's read-only value is 0x6200. Prior to
36e8164882ca,
we interpret that as a 512-byte BAR based on the lowest-order bit set. Per
datasheet sec 5.6.1, that BAR (MFGPT) requires only 64 bytes; use that to
avoid clearing any address bits if a platform uses only 64-byte alignment.

ARM: 8299/1: mm: ensure local active ASID is marked as allocated on rollover
Commit
e1a5848e3398 ("ARM: 7924/1: mm: don't bother with reserved ttbr0
when running with LPAE") removed the use of the reserved TTBR0 value
for LPAE systems, since the ASID is held in the TTBR and can be updated
atomicly with the pgd of the next mm.
Unfortunately, this patch forgot to update flush_context, which
deliberately avoids marking the local active ASID as allocated, since we
used to switch via ASID zero and didn't need to allocate the ASID of
the previous mm. The side-effect of this is that we can allocate the
same ASID to the next mm and, between flushing the local TLB and updating
TTBR0, we can perform speculative TLB fills for userspace nG mappings
using the page table of the previous mm.
The consequence of this is that the next mm can erroneously hit some
mappings of the previous mm. Note that this was made significantly
harder to hit by
a391263cd84e ("ARM: 8203/1: mm: try to re-use old ASID
assignments following a rollover") but is still theoretically possible.
This patch fixes the problem by removing the code from flush_context
that forces the allocated ASID to zero for the local CPU. Many thanks
to the Broadcom guys for tracking this one down.

spi: imx: use pio mode for i.mx6dl
For TKT238285 hardware issue which may cause txfifo store data twice can only
be caught on i.mx6dl, we use pio mode instead of DMA mode on i.mx6dl.

sched/wait: Remove might_sleep() from wait_event_cmd()
The patch
e22b886a8a43 ("sched/wait: Add might_sleep() checks")
introduced a bug in the raid5 subsystem.
The function raid5_quiesce() (and resize_stripes()) uses the 'cmd'
part to release and acquire a spinlock (so we call the sleep
primitives in atomic context), and therefore we cannot do the
might_sleep() check.
Remove it.

xen-netback: stop the guest rx thread after a fatal error
After commit
e9d8b2c2968499c1f96563e6522c56958d5a1d0d (xen-netback:
disable rogue vif in kthread context), a fatal (protocol) error would
leave the guest Rx thread spinning, wasting CPU time. Commit
ecf08d2dbb96d5a4b4bcc53a39e8d29cc8fef02e (xen-netback: reintroduce
guest Rx stall detection) made this even worse by removing a
cond_resched() from this path.
Since a fatal error is non-recoverable, just allow the guest Rx thread
to exit. This requires taking additional refs to the task so the
thread exiting early is handled safely.

net/mlx4_core: Fix kernel Oops (mem corruption) when working with more than 80 VFs
Commit
de966c592802 (net/mlx4_core: Support more than 64 VFs) was meant to
allow up to 126 VFs. However, due to leaving MLX4_MFUNC_MAX too low, using
more than 80 VFs resulted in memory corruptions (and Oopses) when more than
80 VFs were requested. In addition, the number of slaves was left too high.
This commit fixes these issues.

isdn: off by one in connect_res()
The bug here is that we use "Reject" as the index into the cau_t[] array
in the else path. Since the cau_t[] has 9 elements if Reject == 9 then
we are reading beyond the end of the array.
My understanding of the code is that it's saying that if Reject is 1 or
too high then that's invalid and we should hang up.

sunvnet: set queue mapping when doing packet copies
This patch fixes a bug where vnet_skb_shape() didn't set the already-selected
queue mapping when a packet copy was required. This results in using the
wrong queue index for stops/starts, hung tx queues and watchdog timeouts
under heavy load.

qlge: Fix qlge_update_hw_vlan_features to handle if interface is down
Currently qlge_update_hw_vlan_features() will always first put the
interface down, then update features and then bring it up again. But it
is possible to hit this code while the adapter is down and this causes a
non-paired call to napi_disable(), which will get stuck.
This patch fixes it by skipping these down/up actions if the interface
is already down.
