perf probe: Fix concat_probe_trace_events
'*ntevs' contains number of elements present in 'tevs' array. If there
are no elements in array, 'tevs2' can be directly assigned to 'tevs'
without allocating more space. So the condition should be '*ntevs == 0'
not 'ntevs == 0'.

perf stat: Correct --no-aggr description
Description of --no-aggr in perf-stat man page is outdated. --no-aggr
can also be used while profiling specific set of cpus. For ex,
$ perf stat -e cycles,instructions -C 1-2 --no-aggr -- sleep 1

perf tools: Handle partial AUX records and print a warning
This patch decodes the 'partial' flag in AUX records and prints
a warning to the user, so that they don't have to guess why their
PT traces contain gaps (or missing altogether):
Warning:
AUX data had gaps in it 8 times out of 8!
Are you running a KVM guest in the background?
Trying to be even more helpful, we will detect if the user's kvm driver sets up
exclusive VMX root mode for the entire lifespan of the kvm process:
Reloading kvm_intel module with vmm_exclusive=0
will reduce the gaps to only guest's timeslices.
Note however, that you'll still have gaps in cpu-wide traces even with
vmm_exclusive=0, but the number of gaps will be below 100% (as opposed to the
above example).
Currently this is the only reason for partial records.

perf script: Add 'brstackinsn' for branch stacks
Implement printing instruction sequences as hex dump for branch stacks.
This relies on the x86 instruction decoder used by the PT decoder to
find the lengths of instructions to dump them individually.
This is good enough for pattern matching.
This allows to study hot paths for individual samples, together with
branch misprediction and cycle count / IPC information if available (on
Skylake systems).

Only works when no special branch filters are specified.
Occasionally the path does not reach up to the sample IP, as the LBRs
may be frozen before executing a final jump. In this case we print a
special message.
The instruction dumper piggy backs on the existing infrastructure from
the IP PT decoder.
An earlier iteration of this patch relied on a disassembler, but this
version only uses the existing instruction decoder.

perf/x86/intel/pt: Handle VMX better
Since commit:
1c5ac21a0e ("perf/x86/intel/pt: Don't die on VMXON")
... PT events depend on re-scheduling to get enabled after a VMX session
has taken place. This is, in particular, a problem for CPU context events,
which don't normally get re-scheduled, unless there is a reason.
This patch changes the VMX handling so that PT event gets re-enabled
when VMX root mode exits.
Also, notify the user when there's a gap in PT data due to VMX root
mode by flagging AUX records as partial.
In combination with vmm_exclusive=0 parameter of the kvm_intel driver,
this will result in trace gaps only for the duration of the guest's
timeslices.

perf/core: Add a flag for partial AUX records
The Intel PT driver needs to be able to communicate partial AUX transactions,
that is, transactions with gaps in data for reasons other than no room
left in the buffer (i.e. truncated transactions). Therefore, this condition
does not imply a wakeup for the consumer.
To this end, add a new "partial" AUX flag.

perf/core: Keep AUX flags in the output handle
In preparation for adding more flags to perf AUX records, introduce a
separate API for setting the flags for a session, rather than appending
more bool arguments to perf_aux_output_end. This allows to set each
flag at the time a corresponding condition is detected, instead of
tracking it in each driver's private state.

perf tools: Make perf_event__synthesize_mmap_events() scale
This patch significantly improves the execution time of
perf_event__synthesize_mmap_events() when running perf record on systems
where processes have lots of threads.
It just happens that cat /proc/pid/maps support uses a O(N^2) algorithm to
generate each map line in the maps file. If you have 1000 threads, then you
have necessarily 1000 stacks. For each vma, you need to check if it
corresponds to a thread's stack. With a large number of threads, this can take
a very long time. I have seen latencies >> 10mn.
As of today, perf does not use the fact that a mapping is a stack, therefore we
can work around the issue by using /proc/pid/tasks/pid/maps. This entry does
not try to map a vma to stack and is thus much faster with no loss of
functonality.
The proc-map-timeout logic is kept in case users still want some upper limit.
In V2, we fix the file path from /proc/pid/tasks/pid/maps to actual
/proc/pid/task/pid/maps, tasks -> task. Thanks Arnaldo for catching this.

trace/kprobes: Fix check for kretprobe offset within function entry
perf specifies an offset from _text and since this offset is fed
directly into the arch-specific helper, kprobes tracer rejects
installation of kretprobes through perf. Fix this by looking up the
actual offset from a function for the specified sym+offset.
Refactor and reuse existing routines to limit code duplication -- we
repurpose kprobe_addr() for determining final kprobe address and we
split out the function entry offset determination into a separate
generic helper.

gfs2: Avoid alignment hole in struct lm_lockname
Commit
88ffbf3e03 switches to using rhashtables for glocks, hashing over
the entire struct lm_lockname instead of its individual fields. On some
architectures, struct lm_lockname contains a hole of uninitialized
memory due to alignment rules, which now leads to incorrect hash values.
Get rid of that hole.

qed: Fix interrupt flags on Rx LL2
Before iterating over the the LL2 Rx ring, the ring's
spinlock is taken via spin_lock_irqsave().
The actual processing of the packet [including handling
by the protocol driver] is done without said lock,
so qed releases the spinlock and re-claims it afterwards.
Problem is that the final spin_lock_irqrestore() at the end
of the iteration uses the original flags saved from the
initial irqsave() instead of the flags from the most recent
irqsave(). So it's possible that the interrupt status would
be incorrect at the end of the processing.

qed: Align CIDs according to DORQ requirement
The Doorbell HW block can be configured at a granularity
of 16 x CIDs, so we need to make sure that the actual number
of CIDs configured would be a multiplication of 16.
Today, when RoCE is enabled - given that the number is unaligned,
doorbelling the higher CIDs would fail to reach the firmware and
would eventually timeout.

mlxsw: reg: Fix SPVM max record count
The num_rec field is 8 bit, so the maximal count number is 255. This
fixes vlans not being enabled for wider ranges than 255.

net: Resend IGMP memberships upon peer notification.
When we notify peers of potential changes, it's also good to update
IGMP memberships. For example, during VM migration, updating IGMP
memberships will redirect existing multicast streams to the VM at the
new location.

kprobes: Convert kprobe_exceptions_notify to use NOKPROBE_SYMBOL
commit
fc62d0207ae0 ("kprobes: Introduce weak variant of
kprobe_exceptions_notify()") used the __kprobes annotation to exclude
kprobe_exceptions_notify from being probed. Since NOKPROBE_SYMBOL() is a
better way to do this enabling the symbol to be discovered as being
blacklisted, change over to using NOKPROBE_SYMBOL().

perf powerpc: Choose local entry point with kretprobes
perf now uses an offset from _text/_stext for kretprobes if the kernel
supports it, rather than the actual function name. As such, let's choose
the LEP for powerpc ABIv2 so as to ensure the probe gets hit. Do it only
if the kernel supports specifying offsets with kretprobes.

perf kretprobes: Offset from reloc_sym if kernel supports it
We indicate support for accepting sym+offset with kretprobes through a
line in ftrace README. Parse the same to identify support and choose the
appropriate format for kprobe_events.

perf probe: Factor out the ftrace README scanning
Simplify and separate out the ftrace README scanning logic into a
separate helper. This is used subsequently to scan for all patterns of
interest and to cache the result.
Since we are only interested in availability of probe argument type x,
we will only scan for that.

perf tools: Add 'cgroup_id' sort order keyword
This patch introduces a cgroup identifier entry field in perf report to
identify or distinguish data of different cgroups. It uses the device
number and inode number of cgroup namespace, included in perf data with
the new PERF_RECORD_NAMESPACES event, as cgroup identifier.
With the assumption that each container is created with it's own cgroup
namespace, this allows assessment/analysis of multiple containers at
once.
A simple test for this would be to clone a few processes passing
SIGCHILD & CLONE_NEWCROUP flags to each of them, execute shell and run
different workloads on each of those contexts, while running perf
record command with --namespaces option.
Shown below is the output of perf report, sorted with cgroup identifier,
on perf.data generated with the above test scenario, clearly indicating
one context's considerable use of kernel memory in comparison with
others:

While this is a start, there is further scope of improving this. For
example, instead of cgroup namespace's device and inode numbers, dev
and inode numbers of some or all namespaces may be used to distinguish
which processes are running in a given container context.
Also, scripts to map device and inode info to containers sounds
plausible for better tracing of containers.

perf record: Synthesize namespace events for current processes
Synthesize PERF_RECORD_NAMESPACES events for processes that were running prior
to invocation of perf record. The data for this is taken from /proc/$PID/ns.
These changes make way for analyzing events with regard to namespaces.

blk-mq-sched: don't run the queue async from blk_mq_try_issue_directly()
If we have scheduling enabled, we jump directly to insert-and-run.
That's fine, but we run the queue async and we don't pass in information
on whether we can block from this context or not. Fixup both these
cases.

Introduce a new option to record PERF_RECORD_NAMESPACES events emitted
by the kernel when fork, clone, setns or unshare are invoked. And update
perf-record documentation with the new option to record namespace
events.
Committer notes:
Combined it with a later patch to allow printing it via 'perf report -D'
and be able to test the feature introduced in this patch. Had to move
here also perf_ns__name(), that was introduced in another later patch.

dccp: fix memory leak during tear-down of unsuccessful connection request
This patch fixes a memory leak, which happens if the connection request
is not fulfilled between parsing the DCCP options and handling the SYN
(because e.g. the backlog is full), because we forgot to free the
list of ack vectors.

tun: fix premature POLLOUT notification on tun devices
aszlig observed failing ssh tunnels (-w) during initialization since
commit
cc9da6cc4f56e0 ("ipv6: addrconf: use stable address generator for
ARPHRD_NONE"). We already had reports that the mentioned commit breaks
Juniper VPN connections. I can't clearly say that the Juniper VPN client
has the same problem, but it is worth a try to hint to this patch.
Because of the early generation of link local addresses, the kernel now
can start asking for routers on the local subnet much earlier than usual.
Those router solicitation packets arrive inside the ssh channels and
should be transmitted to the tun fd before the configuration scripts
might have upped the interface and made it ready for transmission.
ssh polls on the interface and receives back a POLL_OUT. It tries to send
the earily router solicitation packet to the tun interface. Unfortunately
it hasn't been up'ed yet by config scripts, thus failing with -EIO. ssh
doesn't retry again and considers the tun interface broken forever.

dccp/tcp: fix routing redirect race
As Eric Dumazet pointed out this also needs to be fixed in IPv6.
v2: Contains the IPv6 tcp/Ipv6 dccp patches as well.
We have seen a few incidents lately where a dst_enty has been freed
with a dangling TCP socket reference (sk->sk_dst_cache) pointing to that
dst_entry. If the conditions/timings are right a crash then ensues when the
freed dst_entry is referenced later on. A Common crashing back trace is:

But there are other backtraces attributed to the same freed dst_entry in
netfilter code as well.
All the vmcores showed 2 significant clues:
- Remote hosts behind the default gateway had always been redirected to a
different gateway. A rtable/dst_entry will be added for that host. Making
more dst_entrys with lower reference counts. Making this more probable.
- All vmcores showed a postitive LockDroppedIcmps value, e.g:
LockDroppedIcmps 267
A closer look at the tcp_v4_err() handler revealed that do_redirect() will run
regardless of whether user space has the socket locked. This can result in a
race condition where the same dst_entry cached in sk->sk_dst_entry can be
decremented twice for the same socket via:
do_redirect()->__sk_dst_check()-> dst_release().
Which leads to the dst_entry being prematurely freed with another socket
pointing to it via sk->sk_dst_cache and a subsequent crash.
To fix this skip do_redirect() if usespace has the socket locked. Instead let
the redirect take place later when user space does not have the socket
locked.
The dccp/IPv6 code is very similar in this respect, so fixing it there too.
As Eric Garver pointed out the following commit now invalidates routes. Which
can set the dst->obsolete flag so that ipv4_dst_check() returns null and
triggers the dst_release().

net: use net->count to check whether a netns is alive or not
The previous idea was to check whether a net namespace is in
net_exit_list or not. It doesn't work, because net->exit_list is used in
__register_pernet_operations and __unregister_pernet_operations where
all namespaces are added to a temporary list to make cleanup in a error
case, so list_empty(&net->exit_list) always returns false.

bridge: drop netfilter fake rtable unconditionally
Andreas reports kernel oops during rmmod of the br_netfilter module.
Hannes debugged the oops down to a NULL rt6info->rt6i_indev.
Problem is that br_netfilter has the nasty concept of adding a fake
rtable to skb->dst; this happens in a br_netfilter prerouting hook.
A second hook (in bridge LOCAL_IN) is supposed to remove these again
before the skb is handed up the stack.
However, on module unload hooks get unregistered which means an
skb could traverse the prerouting hook that attaches the fake_rtable,
while the 'fake rtable remove' hook gets removed from the hooklist
immediately after.

ipv6: avoid write to a possibly cloned skb
ip6_fragment, in case skb has a fraglist, checks if the
skb is cloned. If it is, it will move to the 'slow path' and allocates
new skbs for each fragment.
However, right before entering the slowpath loop, it updates the
nexthdr value of the last ipv6 extension header to NEXTHDR_FRAGMENT,
to account for the fragment header that will be inserted in the new
ipv6-fragment skbs.
In case original skb is cloned this munges nexthdr value of another
skb. Avoid this by doing the nexthdr update for each of the new fragment
skbs separately.
This was observed with tcpdump on a bridge device where netfilter ipv6
reassembly is active: tcpdump shows malformed fragment headers as
the l4 header (icmpv6, tcp, etc). is decoded as a fragment header.

ipv6: make ECMP route replacement less greedy
Commit
27596472473a ("ipv6: fix ECMP route replacement") introduced a
loop that removes all siblings of an ECMP route that is being
replaced. However, this loop doesn't stop when it has replaced
siblings, and keeps removing other routes with a higher metric.
We also end up triggering the WARN_ON after the loop, because after
this nsiblings < 0.
Instead, stop the loop when we have taken care of all routes with the
same metric as the route being replaced.

perf: Add PERF_RECORD_NAMESPACES to include namespaces related info
With the advert of container technologies like docker, that depend on
namespaces for isolation, there is a need for tracing support for
namespaces. This patch introduces new PERF_RECORD_NAMESPACES event for
recording namespaces related info. By recording info for every
namespace, it is left to userspace to take a call on the definition of a
container and trace containers by updating perf tool accordingly.
Each namespace has a combination of device and inode numbers. Though
every namespace has the same device number currently, that may change in
future to avoid the need for a namespace of namespaces. Considering such
possibility, record both device and inode numbers separately for each
namespace.

writeback: fix memory leak in wb_queue_work()
When WB_registered flag is not set, wb_queue_work() skips queuing the
work, but does not perform the necessary clean up. In particular, if
work->auto_free is true, it should free the memory.
The leak condition can be reprouced by following these steps:

Above will result in a wb_queue_work() call on an unregistered wb and
thus leak memory.

bpf: improve read-only handling
Improve bpf_{prog,jit_binary}_{un,}lock_ro() by throwing a
one-time warning in case of an error when the image couldn't
be set read-only, and also mark struct bpf_prog as locked when
bpf_prog_lock_ro() was called.
Reason for the latter is that bpf_prog_unlock_ro() is called from
various places including error paths, and we shouldn't mess with
page attributes when really not needed.
For bpf_jit_binary_unlock_ro() this is not needed as jited flag
implicitly indicates this, thus for archs with ARCH_HAS_SET_MEMORY
we're guaranteed to have a previously locked image. Overall, this
should also help us to identify any further potential issues with
set_memory_*() helpers.

selftests/bpf: fix broken build
Recent merge of 'linux-kselftest-4.11-rc1' tree broke bpf test build.
None of the tests were building and test_verifier.c had tons of compiler errors.
Fix it and add #ifdef CAP_IS_SUPPORTED to support old versions of libcap.
Tested on centos 6.8 and 7

mpls: Do not decrement alive counter for unregister events
Multipath routes can be rendered usesless when a device in one of the
paths is deleted. For example:
$ ip -f mpls ro ls
100
nexthop as to 200 via inet 172.16.2.2 dev virt12
nexthop as to 300 via inet 172.16.3.2 dev br0

nexthop as to 201 via inet6 2000:2::2 dev virt12
nexthop as to 301 via inet6 2000:3::2 dev br0
$ ip li del br0
When br0 is deleted the other hop is not considered in
mpls_select_multipath because of the alive check -- rt_nhn_alive
is 0.
rt_nhn_alive is decremented once in mpls_ifdown when the device is taken
down (NETDEV_DOWN) and again when it is deleted (NETDEV_UNREGISTER). For
a 2 hop route, deleting one device drops the alive count to 0. Since
devices are taken down before unregistering, the decrement on
NETDEV_UNREGISTER is redundant.

xen-netback: fix race condition on XenBus disconnect

mpls: Send route delete notifications when router module is unloaded
When the mpls_router module is unloaded, mpls routes are deleted but
notifications are not sent to userspace leaving userspace caches
out of sync. Add the call to mpls_notify_route in mpls_net_exit as
routes are freed.

Make IP 'forwarding' doc more precise
It wasn't clear if the 'forwarding' setting needs to be enabled on the
interface that packets are received from, or on the interface that
packets are forwarded to, or both.
In fact (according to my code reading) the setting is relevant on the
interface that packets are received from, so this change updates the doc
to say that.

netvsc: handle select_queue when device is being removed
Move the send indirection table from the inner device (netvsc)
to the network device context.
It is possible that netvsc_device is not present (remove in progress).
This solves potential use after free issues when packet is being
created during MTU change, shutdown, or queue count changes.

x86/tlb: Fix tlb flushing when lguest clears PGE
Fengguang reported random corruptions from various locations on x86-32
after commits
d2852a224050 ("arch: add ARCH_HAS_SET_MEMORY config") and
9d876e79df6a ("bpf: fix unlocking of jited image when module ronx not set")
that uses the former. While x86-32 doesn't have a JIT like x86_64, the
bpf_prog_lock_ro() and bpf_prog_unlock_ro() got enabled due to
ARCH_HAS_SET_MEMORY, whereas Fengguang's test kernel doesn't have module
support built in and therefore never had the DEBUG_SET_MODULE_RONX setting
enabled.
After investigating the crashes further, it turned out that using
set_memory_ro() and set_memory_rw() didn't have the desired effect, for
example, setting the pages as read-only on x86-32 would still let
probe_kernel_write() succeed without error. This behavior would manifest
itself in situations where the vmalloc'ed buffer was accessed prior to
set_memory_*() such as in case of bpf_prog_alloc(). In cases where it
wasn't, the page attribute changes seemed to have taken effect, leading to
the conclusion that a TLB invalidate didn't happen. Moreover, it turned out
that this issue reproduced with qemu in "-cpu kvm64" mode, but not for
"-cpu host". When the issue occurs, change_page_attr_set_clr() did trigger
a TLB flush as expected via __flush_tlb_all() through cpa_flush_range(),
though.
There are 3 variants for issuing a TLB flush: invpcid_flush_all() (depends
on CPU feature bits X86_FEATURE_INVPCID, X86_FEATURE_PGE), cr4 based flush
(depends on X86_FEATURE_PGE), and cr3 based flush. For "-cpu host" case in
my setup, the flush used invpcid_flush_all() variant, whereas for "-cpu
kvm64", the flush was cr4 based. Switching the kvm64 case to cr3 manually
worked fine, and further investigating the cr4 one turned out that
X86_CR4_PGE bit was not set in cr4 register, meaning the
__native_flush_tlb_global_irq_disabled() wrote cr4 twice with the same
value instead of clearing X86_CR4_PGE in the first write to trigger the
flush.
It turned out that X86_CR4_PGE was cleared from cr4 during init from
lguest_arch_host_init() via adjust_pge(). The X86_FEATURE_PGE bit is also
cleared from there due to concerns of using PGE in guest kernel that can
lead to hard to trace bugs (see
bff672e630a0 ("lguest: documentation V:
Host") in init()). The CPU feature bits are cleared in dynamic
boot_cpu_data, but they never propagated to __flush_tlb_all() as it uses
static_cpu_has() instead of boot_cpu_has() for testing which variant of TLB
flushing to use, meaning they still used the old setting of the host
kernel.
Clearing via setup_clear_cpu_cap(X86_FEATURE_PGE) so this would propagate
to static_cpu_has() checks is too late at this point as sections have been
patched already, so for now, it seems reasonable to switch back to
boot_cpu_has(X86_FEATURE_PGE) as it was prior to commit
c109bf95992b
("x86/cpufeature: Remove cpu_has_pge"). This lets the TLB flush trigger via
cr3 as originally intended, properly makes the new page attributes visible
and thus fixes the crashes seen by Fengguang.

blk: Ensure users for current->bio_list can see the full list.
Commit
79bd99596b73 ("blk: improve order of bio handling in generic_make_request()")
changed current->bio_list so that it did not contain *all* of the
queued bios, but only those submitted by the currently running
make_request_fn.
There are two places which walk the list and requeue selected bios,
and others that check if the list is empty. These are no longer
correct.
So redefine current->bio_list to point to an array of two lists, which
contain all queued bios, and adjust various code to test or walk both
lists.
