vmcoreinfo: add "VMCOREINFO_" to all the call for vmcoreinfo_append_str()

vmcoreinfo: add "VMCOREINFO_" to all the call for vmcoreinfo_append_str()

For readability, all the calls to vmcoreinfo_append_str() are changed to macros
having a prefix "VMCOREINFO_".
