V4L/DVB (3268): Use combined firmware for DVB-T and ATSC Bluebird FusionHDTV USB models.

- Now using firmware "dvb-usb-bluebird-01.fw",
  with combined support for atsc and dvb-t Bluebird models.
- Use usb alt setting 0 for EP4 transfer (dvb-t),
- Use usb alt setting 7 for EP2 transfer (atsc) */
