mutex: Move ww_mutex definitions to ww_mutex.h

mutex: Move ww_mutex definitions to ww_mutex.h

Move the definitions for wound/wait mutexes out to a separate
header, ww_mutex.h. This reduces clutter in mutex.h, and
increases readability.
