Documentation: add meta-documentation for Sphinx and kernel-doc

Describe Sphinx, reStructuredText, the kernel-doc extension, the
kernel-doc structured documentation comments, etc.

The kernel-doc parts are based on kernel-doc-nano-HOWTO.txt, by Tim
<twaugh@redhat.com>.
